Development of project documentation ( construction and executive supervision) and performing author's supervision for investment tasks divided into 13 parts (tasks)-/ Skawina, Krakow, Wieliczka, Szarow, Niepołomice / sectoral order

1. Task No. 1, the scope of which includes the reconstruction of the existing PE gas pipeline dn75-90 on PE100 RC dn 225 in Skawina along Bolesława Śmiałego and Bogucianka streets towards ul. Tyniecka Stage I, in accordance with the Technical Conditions of PSGKR.ZMSM.763.643342.1.18 of 12.3.2018

2. The cost of performance includes:

1) madeie construction project, executive project and a robot project, including: - Obtaining extracts and extracts from land registry maps, - Obtaining agreements / consents of landowners, through which the designed gas network will run, - Implementation of situational and altitude maps for project purposes, - Implementation of a construction project implementing the reconstruction of the gas pipeline according to the above technical conditions, issued bywith Polska Społka Gazownictwa Sp. z o.o. Oddział Zakład Gazowniczy w Krakowie,

Preparing a schedule of design works, which should be approved by the Employer, hereinafter referred to as the Project,

2) obtaining on behalf of and for the benefit of the Contracting Authority, the final decision on the building permit, after which the Contractor will collect and will provide the Employer with a construction log for the investmentand,

3) obtaining all required opinions, permits and decisions necessary for the completeness of the study (ie the water-legal survey, arrangements with PKP, opinion of the archaeologist, geotechnical opinion (including geotechnical investigations), ULI decision (if in accordance with the law is required), arrangements and decisions related to the Natura 2000 program)

4) execution of investor cost estimates and aforementionedrow robot,

5) exercising author's supervision.

I. Contractors who meet the following conditions for participation in the proceedings can apply for the award of the contract:

1. they are not subject to exclusion,

2. meet the following conditions for participation in the proceedings:

2.1 are in the appropriate economic and financial situation,

2.2. From the proceedings after the order is awarded, the Contracting Authority may exclude the Contractor.and the attitude of the premises specified in art. 24 sec. 5 of the Act

2.3. Evaluation of compliance with the above the conditions will be fulfilled according to the formula "meets the requirements", based on the information contained in the declarations and documents to be provided by the Contractor in order to confirm compliance with the conditions of participation in the proceedings. II List of declarations and documents to be provided by the Contractor in order to confirmation-meeting the conditions for participation in the proceedings and the lack of grounds for exclusion.

1. The Contractor shall enclose to the offer the valid statement in the form of a Single Document on the day of submitting the offers

2. Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion and fulfillment in relation to them, to the extent in which they refer to their resources, the conditions of participation in, also provides Uniform GOOD Documents regarding these entities

3. The Contractor who intends to entrust the execution of the Part of the contract to subcontractors in order to demonstrate the absence of grounds for exclusion against them, submits Uniform GOOD Documents regarding these entities

4. In the event of a joint application by the Contractors, the Single Document of FOOD shall be submitted by each of the Contractorjointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion to the extent that each of the Contractors demonstrates fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion. If it is necessary to ensure the proper conduct of the procurement procedure, the Employer may at any stagee proceedings call for the Contractors to submit all or some documents confirming that they are not subject to exclusion, meet the conditions for participation in the proceedings or selection criteria, and if there are reasonable grounds to consider that the previously submitted statements or documents are no longer valid, submit current statements or documents.

6. Proof of bringing a bid bond.

Note. IN-If the Contractors jointly apply for the award of a contract, they are required to present a document establishing a proxy authorized to represent in the proceeding or to represent in the procedure and conclusion of the public procurement contract. The Contractors who meet the following conditions of participation can apply for awarding the contract. proceeding:

- findin an appropriate economic and financial situation / have economic and financial capacity, that is, they are insured against civil liability in respect of their activities related to the subject of the order with a guarantee sum of at least PLN 200, 000 (in words: two hundred thousand zlotys) regardless of the number of Parts, the Contractor is applying for the order.

The contracting party, prior to the order being awarded, will call the Contractor whose tender has been best assessed, to submit, within the deadline not shorter than 10 days, valid as of the day of submitting declarations or documents confirming the circumstances referred to in art. 25 ust. 1 this is: - - the policy, and in the absence of any other document confirming that the Contractor is insured against the answercivil liability in the scope of conducted activity related to the subject of the contract, at least for the amount specified in point 7.1.4. SIWZ

The Contracting Authority, prior to the order being awarded, will call the Contractor whose offer has been best assessed, to submit within the prescribed, not shorter than 10 days, the current date as of the day of submitting statements or documents confirming the circumstances of whichreferred to in art. 25 ust. 1 of the Act, it is:

1. information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Act and regarding convictions for offenses to the penalty of arrest, to the extent specified by the Ordering Party on the basis of art. 24 sec. 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submission of tenders, subject to the content of art. 133 usVol. 4 of the Act,

2. certificate of the competent head of the tax office confirming that the Contractor is not in arrears with payment of taxes, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has concluded an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, inIt obtained legality of exemption, postponement or spreading overdue payments into installments, or suspension of the execution of the decision of the competent authority in full,

3. a certificate issued by the relevant organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with the payment of insurance premiumssocial or health protection, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular obtained the legally required exemption, postponement or payment of overdue payments or installments in installmentskeeping the implementation of the decision of the competent authority in full,

4. excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under art. 24 sec. 5 point 1 of the Act,

5. the Contractor's statement that he has not been the subject of a final judgment of the court or the latteran administrative decision on defaulting on payment of taxes, fees or social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming the payment of such claims together with any interest or fines or entering into a binding agreement regarding repayment of such receivables,

6. Contractor's statement on the lack of a decisionto him as a preventive measure for the prohibition of applying for a public order,

7. the Contractor's statement about the lack of a final conviction of a conviction court for a restriction of liberty or a fine to the extent specified by the Awarding Entity pursuant to Article 24 (5) (5) and (6) of the Act,

8. the Contractor's statement that the final decision of the administration was not issued to himon the violation of obligations arising from the provisions of labor law, environmental law or social security regulations in the scope specified by the Employer pursuant to Art. 24 sec. 5 point 7 of the Act,

9. Contractor's statement on not paying taxes and local fees, referred to in the Act of January 12, 1991 on local taxes and charges (text 1)olity Journal U. of 2016, item 716 with amendments) - The Contractor is obliged to submit a statement of affiliation or non-affiliation to the same capital group within 3 days from the date of posting on the website Ordering party with the opening information.
