Execution of project-estimate documentation for the construction of a medium-pressure gas pipeline with connectiongas in msc. Osielsko, ul. Leśna dz. 206/20

Material scope:

- gas pipeline to DN 63 PE, L = 175 m

- gas connection dn 32 PE, L = 10 m pcs. 1 st. Leśna 206/20

The subject of the orders also includes:

a) providing, upon every request of the Contracting Authority, expert services on the subject and value of the order in each procurement procedure for the performance of construction works indicated in the project documentationof that. In particular, to: promptly prepare a project for answers to questions about the subject of the Order or its estimated value, provide the Ordering Party with written request in the subject matter, make any changes and additions project documentation indicated in the subject of the contract, at the stage of preparation and conduct of any proceedings regarding the poviatno orders for the execution of construction works indicated in the above project documentation within the deadline set by the Employer. The time limit for the performance of the services for the subject matter and the value of the order in the procurement procedure for the construction works indicated in the given documentation covers the period from the day of receipt of the subject of the Contract until the conclusion of the Procurement Contract.and (b) registration of the Construction Journal,

c) update of the investor's cost estimate at the request of the Employer forwarded during the term of the building permit or application,

d) exercise of the author's supervision during the robot construction based on project documentationą,

e) Obtaining all required opinions, permits and decisions, including on behalf of and for the benefit of the Awarding Entity, from the competent authority, the final decision on the building permit.

Project documentation, in terms of each task, should consist of:

a ) construction project (without the documents contained in item b) 2 copies in paper form,

b) construction project (formal part, i.e. consent, declarations, -contracts of owners of plots through which the gas network runs, extracts from the land register) 2 copies in paper form,

c) executive design 3 copies in paper form,

d) matter robot 1 copy in paper form,

e) cost estimate inwestorskiego 1 copy in paper form

f) traffic organization project - 2 copies in paper form

Additionally, the original of the complete dock should be providedumentacji in electronic form on CD / DVD in the file format * .pdf.

Contractors, who:

1 may apply for the order. they are not subject to exclusion,

2. meet the following conditions for participation in the proceedings:

2.1. they have competences or licenses to conduct a specific professional activity, if it results from separate regulations - the Employer does not describe how to do it-assessment of compliance with this condition.

2.2. they have technical or vocational abilities - specified in point III.1.3) announcements - 2.3. they are in an appropriate economic and financial situation - specified in point III.1.2) announcements - 3. The ordering party excludes the Contractor from the proceedings for granting the order based on the premises specified in art. 24 sec. 1 point 12) -23) Pzp and art. 24 sec. 5 point 1) -8) PPL, subject to art. 133 ust. 4 Pzp and art. 24 sec. 7-12 Pzp.

4. A list of mandatory statements and documents submitted together with the offer, confirming compliance with the conditions for participation in the proceedings and no grounds for exclusion:

4.1. Bidding form - according to Annex No. 2 to the ToR,

4.2 Statement in the form of a single document, to the extent indicated in the ToR,

4.3 The contractor that is set upthey are committed to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and fulfillment to them, to the extent that they refer to their resources, the condition of participation in the proceedings, also submits uniform documents concerning these entities, signed by the entity whose documents concern,

4.4. In the event of joint application for contractors, a single documenteach of the contractors jointly applying for the order is in charge. These documents confirm compliance with the conditions of participation in the selection procedure or criteria and no grounds for exclusion to the extent that each contractor meets the conditions for participation in the selection procedure or criteria and no grounds for exclusion,

4.5 If the contractor indicates that part of the order will be executedby a subcontractor, submit a single subcontractor's document. In this case, a single document should refer to the verification of the grounds for exclusion,

4.6 If the Contractor is represented by the Proxy, the offer should be accompanied by the original power of attorney, unless the authorization is based on other documents attached to the offer.him condition.

1. The contracting party, prior to the order being awarded, will call the contractor whose offer has been best assessed, to submit within the prescribed period, not shorter than 10 days, valid as of the day of submitting declarations or documents confirming the circumstances referred to in art. 25 ust. 1 of the Act, that is:

1.1 documents listed in point 8.2. Terms of Reference.

1.2. information from the National Register of Kin the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Act and regarding a conviction for offenses to a prison detention, to the extent specified by the ordering party pursuant to art. 25 section 5 items 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting bids.

Subject to the content of art. 133 ust. 4 of the Act, the contractor is not subject to exclusion in the case referred to in-Art. 24 sec. 1 point 13 lit. d of the Act and in the case referred to in art. 24 sec. 1 point 14, if the person referred to in this provision has been convicted of an offense listed in art. 24 sec. 1 point 13 lit. d of the Act.

1.3. certificate of the head of the competent tax office confirming that the contractor is not in arrears with paying taxes, issued not earlier than 3 months before the expiry ofthe submission of bids, or other document confirming that the contractor has concluded an agreement with the competent tax authority regarding the repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or installment payment of overdue payments or suspension in full of the implementation of the decision authority;

1.4. certificate of ownershipthe off-site organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the contractor has concluded agreement with-the competent authority regarding the repayment of such receivables, together with any interest or fines, in particular obtaining the legally required exemption, deferral or payment in installments of overdue payments or suspension in full of the implementation of the decision of the competent authority;

1.5. excerpt from the relevant register or from central records and information on economic activity, if separate regulations are requiredthey are entering an entry in the register or records in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act;

1.6. the contractor's statement that he has not been the subject of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or - in the event of such a decision or decision -documents confirming payment of these receivables together with any interest or fines, or conclusion of a binding agreement regarding repayment of such receivables;

1.7. the contractor's statement about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders;

1.8. the contractor's statement of non-payment of taxes and local fees, oreferred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716),

1.9. the contractor's statement on the lack of a final conviction of a conviction court for offense of restriction of liberty or a fine to the extent specified by the awarding entity pursuant to art. 24 sec. 5 points 5 and 6 of the Act;

1.10. the contractor's statement that no final decree was issued against himadministrative law on violation of obligations resulting from the provisions of labor law, environmental protection law or social security provisions in the scope determined by the contracting party pursuant to art. 24 sec. 5 point 7 of the Act
