Preparation of project and estimate documentation for the construction of the gas networkthe same day with gas connections in the months of. Golub-Dobrzyń, ul. Kamienna 9

Material scope: - - gas pipeline de 63 PE L = 600.00 mb

- vacuum connection de 32 PE L = 30 m 2 • The subject of the orders also includes:

a) performance upon each call Employer of expert services on the subject and value of the order in each procurement procedure for the performance of construction works indicated in the documentproject experience. In particular to: immediate preparation of the project answers to questions about the subject of the Order or its estimated value, providing the Ordering Party's opinion in writing in the subject, making any changes and additions to the project documentation indicated in the subject of the contract, at the stage of preparing and conducting eachrequest for a contract for construction works indicated in the above project documentation within the deadline set by the Employer. The time limit for the performance of services for the subject matter and the value of the order in the procurement procedure for performing construction works indicated in the given documentation covers the period from the date of receipt of the subject of the Contract until the conclusion of the Agreement in case(b) registration of the Construction Journal,

c) update of the investor's cost estimate at the request of the Employer forwarded during the validity of the building permit or application,

d) exercise of author's supervision in the during the construction works carried out on the basis of the completed documentationdesignation,

e) obtaining all required opinions, permits and decisions, including on behalf of and for the benefit of the Employer, from the competent authority, the final decision on the building permit.

Design documentation, in terms of each task, should consist of: a) construction project (without the documents contained in item b) 2 copies in paper form,

b) construction project (formal part, i.e. consent, declarations, landowners' agreements through the gas network, extracts from the land register) 2 copies in paper form,

c) executive design, 3 copies in paper form,

d) matter, robot 1 copy in paper form,

e) investor's cost estimate 1 copy in paper form

f) traffic organization project - 2 copies in paper form

Additionally, the original should be provided-complete documentation in electronic form on CD / DVD in the file format * .pdf.

Contractors, who:

1 may apply for the order. they are not subject to exclusion,

2. meet the following conditions for participation in the proceedings:

2.1. they have competences or licenses to conduct a specific professional activity, if it results from separate regulations - the Employer does not describe the wayboth making an assessment of compliance with this condition.

2.2. they have technical or vocational abilities - specified in point III.1.3) announcements

2.3. they are in an appropriate economic and financial situation - specified in point III.1.2) announcements - 3. The ordering party excludes the Contractor from the proceedings for granting the order based on the premises specified in art. 24 sec. 1 point 12) -23) Pzp and art. 24 ust. 5 pts 1) -8) Pzp, subject to art. 133 ust. 4 Pzp and art. 24 sec. 7-12 Pzp.

4. A list of mandatory statements and documents submitted together with the offer, confirming compliance with the conditions for participation in the proceedings and no grounds for exclusion:

4.1. Bidding form - according to Annex No. 2 to the ToR,

4.2 Statement in the form of a single document, to the extent indicated in the ToR,

4.3 Performer, which refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and fulfillment to them, to the extent that they refer to their resources, the condition of participation in the proceedings, also submits uniform documents concerning these entities, signed by the entity whose documents concern,

4.4 In the case of joint application for contractors, uniformthe document is submitted by each of the contractors jointly applying for the order. These documents confirm fulfillment of the conditions of participation in the selection procedure or criteria and the lack of grounds for exclusion to the extent that each of the contractors demonstrate compliance with the conditions for participation in the selection procedure or criteria and no grounds for exclusion.

4.5 If the contractor indicates that part of the order will beit is made by a subcontractor, submits a single subcontractor's document. In this case, a single document should refer to the verification of the grounds for exclusion. 4.6 If the Contractor is represented by the Proxy, the offer should be accompanied by the original power of attorney, unless the authorization is based on other documents attached to the offer.-fulfilling this condition.

1. The contracting party, prior to the order being awarded, will call the contractor whose offer has been best assessed, to submit within the prescribed period, not shorter than 10 days, valid as of the day of submitting declarations or documents confirming the circumstances referred to in art. 25 ust. 1 of the Act, that is:

1.1 documents listed in point 8.2. Terms of Reference.

1.2. information from countriesego Register of Criminal Records in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Act and regarding a conviction for offenses to a prison detention, to the extent specified by the ordering party pursuant to art. 25 ust. 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting bids.

Subject to the content of art. 133 ust. 4 of the Act, the contractor is not subject to exclusion in the casereferred to in art. 24 sec. 1 point 13 lit. d of the Act and in the case referred to in art. 24 sec. 1 point 14, if the person referred to in this provision has been convicted of an offense listed in art. 24 sec. 1 point 13 lit. d of the Act.

1.3. certificate of the head of the competent tax office confirming that the contractor is not in arrears with paying taxes, issued not earlier than 3 months-before the deadline the submission of bids, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible exemption, deferment or installment payment of overdue installments or suspension in full decision of the competent authority;

1.4. andprovide the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued no earlier than 3 months before the deadline for submission of tenders, or other document confirming that the contractor concluded-agreement with the competent authority regarding the repayment of such receivables, together with any interest or fines, in particular obtained legally required exemption, postponement or payment in installments of overdue payments or suspension in full of the implementation of the decision of the competent authority;

1.5. excerpt from the relevant register or central records and business information, if separatee the provisions require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act;

1.6. the contractor's statement that he has not been the subject of a final judgment of the court or a final administrative decision on late payment of taxes, fees or social security or health insurance premiums, or - in the event of such a judgment being passed-or decisions - documents confirming the payment of these receivables together with any interest or fines, or conclusion of a binding agreement regarding repayment of such receivables;

1.7. the contractor's statement about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders;

1.8. the contractor's statement of non-payment of taxes and feeslocalities, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716),

1.9. the contractor's statement about the lack of a final conviction of a conviction court for offense of restriction of liberty or a fine to the extent specified by the awarding entity pursuant to art. 24 sec. 5 points 5 and 6 of the Act;

1.10. the contractor's statement that he has not been handed over to himthe final administrative decision on violation of obligations resulting from the provisions of labor law, environmental protection law or social security regulations in the scope specified by the awarding entity on the basis of art. 24 sec. 5 point 7 of the Act
