Endoscopic, in the assortment specified in Annex 1 to the ToR. All offered accessories must be suitable for gastroscopes with a minimum internal diameter of 2.8 mm and colonoscopes with an internal diameter of 3.7 mm and duodenoscopes with an internal canal diameter4.2 mm).

The subject of the order is the supply of accessories for endoscopic procedures, in the assortment specified in Annex 1 to the ToR. All offered accessories must be suitable for gastroscopes with a minimum internal canal diameter2.8 mm and colonoscopes with an internal diameter of 3.7 mm and duodenoscopes with an internal diameter of 4.2 mm).

The proposed subject of the order must have all the documents required by Polish law, on the basis of which it may be admitted to trading and used in health care facilities in the Republic of Poland.

Supplied accessories must have at the time of deliveryto the Ordering Party minimum 12-month shelf-life.

The Contractor may be requested to submit samples at the request of the Employer.

5.1. Contractors, who:

1) can not be excluded from the procedure for awarding a public order pursuant to art. 24 sec. 1 item 12-23 and pursuant to art. 24 sec. 5 points 1 and 8 of the Act.

Information from the Countrythe Criminal Record in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids or applications for admission to participation in the proceedings. - Certificate of the competent head of the tax office confirming that the contractor is not in arrears with paying taxes, issued no earlier than 3 months before the end of terminu of submitting bids, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally required exemption, deferral or installment payment of overdue payments or suspension in full execution of the decision of the competent authority. to confirm the lackgrounds for exclusion under art. 24 sec. 5 point 8 of the Act - Certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submissionbids, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of such receivables with any interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension of the full implementation of the decision of the competent authority. to confirm the absence of grounds for exclusion onbased on art. 24 sec. 5 point 8 of the Act.

Contractor`s statements:

a. about the failure to issue a final court judgment against him or a final decision of the adminon the default of taxes, social security or health insurance premiums or, in the event of such a judgment or decision being issued, documents confirming the payment of such claims together with any interest or fines or the conclusion of a binding agreement to repay those amounts; on the lack of a ruling against him as a preventive measure andbefore applying for a public order;

c. on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716) - Information materials on the subject of the order, eg: folders, catalogs, leaflets, pictures, descriptions (in Polish), where it should be clearly marked which range of mate is offeredThey concern (task number, item number of the Assortment and Price Form, catalog number / Ref.),

- Certificates of the entity authorized to control quality confirming that the delivered products meet specific standards or technical specifications:

a) Contractor`s declaration about the class of medical device (class I, II a, II b, or III) if applicable;

b) with regard to medical devices i.e .: O.provision of possession of the documents required by the Act of 20.5.2010 on medical devices (Journal of Laws of 2010 No. 107, item 679 with later amendments) allowing admission of medical devices to the market and an obligation to provide them for each Ordering Party`s request - Appendix No. 7 to the ToR.

The required statement applies to medical devices. In case the product is not a medical device andnot subject to the provisions of the Act of 20.5.2010 on medical devices or European Directive 93/42 / EEC, the Employer requires submission of a statement that these documents are not required (mentioning in a statement that a given product is not a medical device) - Annex No. 7 to the SIWZ.
