Delivery and assembly of the test stand assessment of potential injuries to the body in the aspect of fire or impact. Supply and assembly of the test stand for evaluation of potential injuriesin the aspect of shelling or striking.

The Procurer will exclude Contractors in the circumstances referred to in Art. 24 section 1 items 12-23 and para. 5 point 1 of the Public Procurement Law.

According to art. 13 para. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 on the protection of natural persons in connection with the processingiem personal data and on the free flow of such data and the repeal of Directive 95/46 / EC (general regulation on data protection) (Official Journal L 119 of 04.05.2016, page 1), hereinafter "RODO", I inform that :

Your personal data will be processed pursuant to Article 6 (1) (c) of the GDPR for purposes related to the proceedings for this public contract; - Your recipients of your personal data will be a personto or entities to which the documentation of proceedings based on art. 8 and art. 96 para. 3 of the Act of 29.1.2004 Public Procurement Law (Journal of Laws of 2017, item 1579 and 2018), hereinafter the "PZP Act", your personal data will be stored in accordance with Article 97 paragraph 1 of the Public Procurement Law, for a period of 4 years from the date of completion of the procurement procedure, and if the duration-the contract exceeds 4 years, the storage period covers the entire duration of the contract; - The obligation to provide your personal data directly to you is a statutory requirement set out in the Public Procurement Law, related to participation in the public procurement procedure; the consequences of failure to provide specified data result from the Public Procurement Law Act - regarding your data-personal decisions will not be taken in an automated manner, application to art. 22 RODO;

You have:

You are not entitled to:

- in connection with the Article . 17 sec. 3 lit. b, d or e RODO right to delete personal data;

- pursuant to art. 21 RODO right to object to the processing of personal data, as the legal basis for the processing of your personal data is art. 6 par. 1 lit. c RODO.

In order to confirm the absence of grounds for exclusion of the Contractor from participation in the proceedings, the Employer requests submission of the following documents: - information from the National Criminal Register in the scope specified in art. 24paragraph. 1, items 13, 14 and 21 of the Act - copy from the relevant register or central records and information on economic activity, if separate regulations require an entry in the register or records, in order to confirm the lack of grounds for exclusion based on art. 24 sec. 5 point 1 of the Public Procurement Law Act - the statement of the contractor about the lack of issuing a final court judgment or final administrative decision to himon the default of taxes, social security or health insurance fees or, in the event of such a judgment or decision, documents confirming the payment of such claims together with any interest or fines or a binding agreement on repayment of such debts;

- the contractor's statement about the lack of a decision against him titlea preventive measure prohibiting applications for public orders;
