Supply of electricity to the headquarters of the Regional Center for Blood Donation and Blood Treatment in Poznań.

Supply of electricity to the headquarters of the RegionThe Center for Blood Donation and Blood Treatment in Poznań will include 2 energy points located in buildings at ul. Marcelińska 44, 60-354 Poznań under the contract, in accordance with the Act of 10.4.1997, Energy Law, along with the implementing provisions to this Act.

Distribution services of electricity will be provided on the basis of a separate agreement concluded with the localm The operator of the Distribution System. The detailed description of the order is contained in Form No. 1 and Annex No. 1 and their possible modifications constituting an integral part of this ToR. contract, according to art. 32 para. 1 of the Energy Law Actne (consolidated text from 2018, item 755, as amended) for conducting business activity in the scope of trading in electricity, issued by the President of the Energy Regulatory Office. The ordering party informs that it does not provide for the possibility of ordering, referred to in art. 67 sec. 1 point 7 and does not provide for a dynamic purchasing system

II. Each offer must be secured with a deposit of 15PLN 000.00, requirements described in Chapter VIII in the SIWZ.

III. Pursuant to Chapter VI of the Terms of Reference:

6. If the Contractor has a registered office or place of residence outside the territory of the Republic of Poland:

6.1. It shall provide information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor is established orthe place of residence or place of residence has a person who is concerned with information or a document, to the extent specified in art. 24 sec. 1 points 13, 14 and 21;

6.2. It places a document or documents issued in the country in which the Contractor has his registered office or place of residence, confirming that: a) he is not in arrears with paying taxes, fees, social security or health insurance contributions or thatarla agreement with the competent authority regarding the repayment of such receivables, together with any interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or withholding in full the decision of the competent authority, b) its liquidation was not opened or announced bankruptcy.

6.3. Documents referred to in point 6.1. and point 6.2point b, should be issued no earlier than 6 months before the deadline for submitting bids or applications for admission to participate in the proceedings. - Document referred to in point 6.2. point a, it should be issued no earlier than 3 months before the deadline expires

6.4. If in the country in which the Contractor has the seat or place of residence or place of residence has a person to whomthe document concerns, no documents referred to in point 6.1., They are replaced by a document containing the Contractor's declaration, indicating the person or persons authorized to represent it, or a statement of the person the document was to be submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authoritydue to the seat or place of residence of the Contractor or place of residence of that person. The provision of point 6.3. applies.

6.5. Contractor established on the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, which is referred to in the document indicated in point 2.1. and 2.2., submits the document referred to in point 6.1., In the fieldreferred to in Article 24 sec. 1 point 14 and 21. If the country in which the person to whom the document was to reside is domiciled, no such documents are issued, it is replaced by a document containing a statement made by the notary or before a judicial, administrative or self-regulatory body, or the economic proper for the place of residence of this oersons. The provision of point 6.2. the first sentence applies.
