Providing services in the field of individual oligophrenopedagogical therapy for children under the comprehensive support program for families "Behind Life"

The subject of the contract is the provision of services in the field of individual oligophrenopedagogical therapy for children under comprehensive support program for families `Behind Life`. estimate ofand the number of hours of the service in the period from the date of the contract to 31/12/2020 was specified in the range of min. 180 max. 240 h.

Main Site: Area of the city of Katowice.

Priority of the II Program of comprehensive support for families "Behind life" early support for the development of the child and his family. The task is financed from the state budget. The subject of the contract is the provision of individual servicesoligophrenopedagogical therapy for children as part of the "For Life" comprehensive family support program. The estimated number of hours of the service from the date of the contract to 31/12/2020 has been specified in the range of at least 180 max. 240 hours.

Oligophrenopedagogical therapy services will be provided in the form of individual classes with children from the moment they are born until they start schoolK-ln. One hour of service is assumed to be a full clock hour (60 minutes). The contractor`s remuneration will be the product of the flat-rate hourly gross remuneration for one hour of the service proposed in the offer and the number of hours of service actually carried out under the program. The amount which the Employer intends to allocate to finance the contract, is PLN 60 gross for one hour of service provision. In total, no more than PLN 14, 400.00 gross for 240 hours.

The scope of the obligations of a specialist conducting individual oligophrenopedagogical therapy for children includes:

- after appointing a team of specialists for a specific child, determining the directions and schedule of actions taken in the field of early support and support for familiesy,

- establishing cooperation with the institution in which the child is covered by other therapeutic activities to ensure consistency of interactions,

- developing and implementing with the child and his family an individual early development support program in the part intended for the oligophrenopedagogue,

- assessing progress and difficulties in the functioning of the child, including identifying and eliminatingbarriers and restrictions in the environment hindering his activity and participation in social life,

- analyzing the effectiveness of assistance provided based on the program to the child and his family,

- introducing changes in the program according to the current needs of the child and his family,

- planning further activities in the field of early support.

Employment requirement under a contract of employment. AwardingThe Contractor requires that the Contractor or the Subcontractor throughout the entire period of performance of the subject of the contract employ under the employment contract within the meaning of the provisions of the Act of 26.6.1974 Labor Code (i.e. Journal of Laws 2019 item 1040, as amended) activities indicated above. The time dimension must correspond to the time necessary for the employees to perform all activities covered by the service of the witnessof individual oligophrenopedagogical therapy.

Criteria for the evaluation of bids:

Price (C) = weight 48%, service availability (D) = weight 52%.

The score under the service availability criterion (D) will be awarded as follows:

Declaration of service in 3 selected locations from the offer form list (minimum required by the Employer) = 0 points

For declaring the provision of services in 1 selected location above the required minimum =4 points The maximum number of points to be obtained under this criterion is 52 points

Note: If the contractor does not declare a minimum of 3 places to perform the service from the list of the offer form, the offer will be rejected.

The total number of P points in the assessment of each offer is the sum of points calculated in its assessment according to each criterion: P = CD
