Priority II of the Comprehensive Family Support Program 'Behind Life' - early support for the development of the child and his family Task financed from the state budget funds. provision of services in the field of individual speech therapy for children as part of a comprehensive support program for familiesin "For Life." Speech therapy services will be provided in the form of individual classes with children from the moment of their birth to the time they take their school education, for one hour of service provision is a full clock hour (60 minutes). determined for this part in the range: 260 h max. 320 h.

The scope of duties of the leading specialistthe individual speech therapy therapy for children includes: - - after appointing a team of specialists for a specific child, setting directions and schedule of measures taken in the field of early support and family support - establishing cooperation with a child care center where other children are covered by therapeutic measures to ensure interaction of interactions

- development and implementation- the assessment of progress and difficulties in the child's functioning, including identifying and eliminating barriers and limitations in the environment that hinder its activity and participation in social life - analyzing the effectiveness of assistance provided to the child and his family based on the program of the child and his-family - introducing changes to the program according to the current needs of the child and his family

- planning further actions in the field of early support

The contractor's remuneration will be the product of a flat-rate hourly gross remuneration for one hour of the service offered in the offer and the number of hours of service actually carried out in within the framework of the programerty is a flat-rate price and includes all costs related to the implementation of the contract (subject of the contract) and necessary for its completion, in particular:

- Cost of transport to the service location

- Costs of materials necessary for the proper provision of the service

- Salary costs for service providers

- insurance costs

- taxes, including VAT calculated in accordance with the applicablemy regulations,

- other fees that may occur in the execution of the order

Proceedings for the order are carried out by the attorney: director of the Shared Service Center in Katowice, acting on behalf of and for the benefit of the Ordering Party: Specialist Psychological and Pedagogical Clinic, Sokolska Street 26, 40 -086 Katowice, Poland, based on the power of attorney granted in accordance with art.15 para. 2 of the Act of 29.1.2004.
