Reference Number: ZP / 994 / 2018 - Priority II of the Comprehensive Support Program for families "For Life" - early development supportju child and his family. Task financed from the funds of the state budget. The subject matter of the order is the provision of services in the field of individual psychological therapy for children as part of a comprehensive support program for families "Za życie." Psychological therapy services will be provided in the form of individual classes with children from the moment of birth to the time of learningfootball. One hour of service provision is assumed to be a full clock hour (60 mins).

Priority II of the Comprehensive Family Support Program 'Behind Life' - early support of the child and his family development. Task financed from the budget The subject of the order is the provision of services in the field of individual psychological therapy for children in-as part of a comprehensive support program for families "For Life." Psychological therapy services will be provided in the form of individual classes with children from the moment of their birth to the time they take their school education, one hour of service provision is a full clock hour (60 minutes). The estimated number of hours has been specified for this part in the range: minimum 260 hmax 320 h.

The scope of duties of a specialist leading individual psychological therapy for children includes: - - after appointing a team of specialists for a specific child, setting directions and schedule of measures taken in the field of early support and family support - establishing a relationship with the child's ward is covered by other therapeutic activities to ensure- interaction and development of the individual program of early development support in the part intended for the psychologist

- assessing and progressing the child's functioning, including identifying and eliminating barriers and limitations in the environment that hinder his or her activity and participation in life. social

- analyzing effectivenessassistance provided on the basis of the program to the child and his family - introduction of changes to the program according to the current needs of the child and his family

- planning of further measures in the field of early support

The contractor's remuneration will be the product of a flat-rate hourly gross remuneration for one hour of the service offered in offer and the number of hours of service-actually carried out under the program. The price of the offer is a flat-rate price and includes all costs related to the implementation of the contract (subject of the contract) and necessary for its completion, in particular:

- Cost of transport to the place where the service is provided

- Cost of materials necessary for the proper provision of the service - Costs of remuneration for persons providing services

- insurance costs

- will provideincluding the VAT calculated in accordance with the applicable regulations,

- other fees that may occur in the execution of the order

Proceedings on the order are led by a proxy: director of the Shared Services Center in Katowice, acting on behalf of and for the benefit of the Ordering Party: Specialist Clinic Psychological Pedagogiczna, ul.Sokolska 26, 40-086 Katowice, Poland, -based on the power of attorney granted in the mode of art. 15 para. 2 of the Act of 29.1.2004.
