• The subject of public procurement is the ultrasound equipment supply for the cardiology clinic of the Clinic of Pediatric and Adolescent Medicine General Faculty Hospital in Prague (VFN), including accessory, customs, transport, installation, putting into operation, performing the instructions according to the relevant provisionAct No. 268/2014 Coll. on Medical Devices (Act No. 268/2014 Coll. training of the purchaser and delivery of the documents relating to the goods to be supplied, especially the declaration of conformity and instructions for use in the Czech language in printed and electronic form, including the guarantee service and further according to the other conditions of the documentation.and ultrasonic equipment for the cardiology clinic of the Clinic of Pediatric and Adolescent Medicine General Faculty Hospital in Prague (VFN), including accessory, customs, transport, installation, putting into operation, execution of instructions according to the relevant provision of Act No. 268/2014 Coll. on Medical Devices (Act No. 268/2014 Coll. training the buyer's handler and handing over the documents, toincluding the declaration of conformity and instructions for use in the Czech language in printed and electronic form, including warranty service and further according to the other conditions of the filing documentation. (a) a record of the trade register, if it is entered in it, or a statement from another similar to the record in which the supplier is registered according to a special legal regulation (Section 77 (1) of the Act); b)as well as its ability to distribute and service medical devices, ie one of the following possibilities: - About the confirmation of fulfillment of the reporting duty on the SLAF or - About the extract from the S-KL registry pursuant to Act No. 268/2014 Coll. on Medical Devices and on Amendment of the Act No. 634/2004 Coll., on Administrative Fees, as amended, or the URL address to which registration is possible.
