The subject of the order is diagnostic imaging technique - ultrasonographic (hereinafter referred to as the USG apparatus), as determined by:. The place of performance is the place of delivery of the subject of the order. The place is filledand healthcare facilities under the management of a public contracting authority in the territory of Bratislava.

The applicant must meet the conditions for the personal status mentioned in - 32 par. 1 of the Act and shall prove them by submitting a copy of the original documents or a certified copy thereof in accordance with Article 32 2, 4 and 5 or pursuant to Section 152 of the Act. Documents must be current and reflect the actual status inthe time when the applicant enters a public contract:

1. The Applicant demonstrates fulfillment of the terms and conditions stated in - 32 par. 1 of the Public Procurement Act with the following documents:

a) letter. (a) a certified extract of the criminal record not older than 3 months on the expiry of the time limit for the submission of tenders. In connection with the acquisition of the Act no. 91/2016 Z. z. on criminal liability of lawyersh persons and on the amendment of certain laws from 1.7.2016, the applicant shall also submit an extract from the criminal record of legal persons not older than 3 months; b) proof of health insurance and Social Insurance Company's confirmation not more than 3 months on the expiration of the deadline for submitting tenders; (c) attested by a local tax office not more than three months before thethe deadline for submission of tenders; (d) attested by a confirmation of the relevant consignment no more than 3 months from the date of expiry of the time limit for submission of tenders; (e) a document proving the right to deliver the goods, to perform construction work or to provide a service which corresponds to the subject of the contract; (f) a certified statement of honor. If I'm a solicitor, a place of business or obhave expired outside the territory of the Slovak Republic and the statutes of the Slovak Republic, the places of business or usual residence do not issue any of the documents mentioned in point 1.1. or they do not issue equivalent documents, they may be replaced by an affidavit in accordance with the rules in force in the state of his or her place of business, place of business or usual residence. If the right of the status quo applies to a sideline, a place of business or habitual residence-outside the territory of the Slovak Republic, it does not regulate the institute of honorary declaration, it may be replaced by a declaration made by a court, a body, a notary, another professional institution or a commercial institution according to the regulations in force in the state of establishment, place of business or usual residence of the applicant. The Applicant is considered as fulfilling the condition of the person's personal status under Section 32-1 letter. b) and c), if he paid the arrears or was authorized to pay arrears in installments. If the tender is submitted by a group of contractors, it proves the fulfillment of the conditions for participation in the public procurement of such personal status within the meaning of - 32 of the Public Procurement Act, for each member of the group separately. Correction to supply the goods or provide a service is demonstrated by a member of the group only in a relationship-to that part of the subject of the order to ensure it. The submission of a scanned copy of the original or a certified true copy of all documents is required (in case of request by the contracting authority, the applicant is required to submit the original documents). The entrant can fulfill the conditions of his / her personal status to be entered in the list of economic subjects in accordance withSection 152 of the Public Procurement Act. The public procurer shall recognize the equivalent entry or registration certificate issued by the competent authority of another Member State by which the tenderer demonstrates that the conditions for participation in the procurement have been fulfilled. The contracting authority shall also accept an equivalent document submitted by the applicant. Entry in the list of entrepreneurs carried out under the Public Procurement Act until 17.4.2016 is entered in the list of economic subjects within the scope of recorded facts. In the event that the entry in the list of economic entities does not cover the conditions for the participation in the personal status provided in - 32 par. (1) of the Act on Public Procurement, the applicant shall prove these facts by means of a separate document proving the required condition of part time issued by the relevant institution.The fulfillment of the conditions of part-time can be demonstrated by submitting the completed single European document (JED) pursuant to - 39 par. 1 of the Act, which the economic entity may provisionally replace the documents proving the fulfillment of the terms and conditions stipulated by the contracting authority.
