Framework Contract Supply and Replacement of Water Filters

c) declaration on the advancement of women in accordance with the regulations of the Berlin Women's Promotion Ordinance gem. Form 7;

d) Self-declaration to the rate loyalty, minimumpay and social security contributions acc. Form 8;

e) Self-declaration for inquiries according to - 6 of the Corruption Register Act (KRG) acc. Form 9;

f) Proof of certification according to DIN EN ISO 13485;

g) (if applicable) Declaration of the consortium in accordance with Form 2 (for details, see Item 10 Conditions of application) and / or subcontractors in accordance with Form 4 (for details, see piff. 11) If not applicable, this must be explicitly stated in the letter of application;

1) Vivantes Network for Health GmbH has commissioned Vivantes Service GmbH to provide all services related to central purchasing for Vivantes Netzwerk f-r Gesundheit GmbH including its subsidiaries to provide. This includes in particulare the preparation and execution of tenders / tendering procedures up to the award of contract / contract award. Vivantes Service GmbH is authorized to act on behalf of and for the account of Vivantes Netzwerk f-r Gesundheit GmbH and its subsidiaries and to conclude contracts for them;

2) Tenderers are obliged to observe collective bargaining agreementsgem. - 1 Abs. 2 Berlin Tendering and Vergabegesetz or for payment minimum wage of 9.0 EUR acc. Section 1 (4) of the Berlin Tendering and Public Procurement Act and the obligation to pass on this obligation to subcontractors. Details result from the Berlin Tendering and Public Procurement Act;
