The subject of the order is catching, admission to the animal shelter and maintenance homeless dogs caught from the area of the commune of Gołdap (estimated number of dogs caught about 60 pcs per year)

The subject of the order is catching, admission to the animal shelter and maintenance of homeless dogs caught from the area of the commune of Gołdap (estimated number of dogs caught about 60 pcs per year) by performing the following actions:

1. Catching, catching and transporting homeless dogs to the shelter with equipment adapted to these purposes, enabling safe and humaniefficient, as well as least burdensome for animals performance of the above operation;

2. Providing homeless dogs caught from the area of the Gołdap commune care in a shelter, in accordance with applicable regulations, including appropriate veterinary care in necessary cases, consisting in particular in providing animals with:

- rooms or boxes so that they can move freely, protecting themagainst cold, heat and precipitation,

- proper treatment and food,

- isolating rooms for aggressive and sick dogs.

3. Performing veterinary services such as: vaccination, deworming, tick-off, sterilization, castration, treatment of sick animals, sleeping blind litters, euthanasia of animals not promising further survival after obtaining the opinion of a doctorand veterinary medicine;

4. Taking action to find the owner and return the owners of dogs placed in the shelter after documenting their rights to the animal;

5. Marking the dog with a microchip 14 days after admitting the dog to the shelter;

6. Keeping records of accepted and released animals (photographic documentation, date, place of harvesting and adoption, etc.the formation on whose behalf the animal was caught, veterinary procedures, microchip number, date of adoption or euthanasia, adoption agreement, etc.);

7. Undertaking activities aimed at acquiring new owners able to provide the animal with the best living conditions. The contractor should generally accept (media announcements, website, social networking sitesinform residents about the shelter and encourage them to adopt dogs. Detailed conditions to be implemented under this contract are set out in Annex 8 of the Terms of Reference. as it results from separate provisions, are possessing:

- authorizedand for conducting activities in the field of running shelters for homeless animals in accordance with the Act of 13.9.1996 on maintaining cleanliness and order in municipalities (i.e., Journal of Laws of 2018, item 1454, as amended),

- decision of the competent local Poviat Veterinary Officer to assign an identification number to the veterinary shelter, in accordance with the Act of 11.3.2004 on health protection related toand the control of infectious diseases (i.e.: Journal of Laws of 2018, item 1967 as amended,

- permits for the carrier and certificate of approval of means of road transport used for long-distance transport of animals, referred to in the provisions of Council Regulation (EC) No. 1/2005 of 22.12.2004 on the protection of animals during transport and related operations, and amending dDirectives 64/432 / EEC and 93/119 / EC and Regulation (EC) No 1255/97, - driver and attendant licenses in accordance with Article 17 clause 2 Council Regulation (EC) No 1/2005 of 22.12.2004 on the protection of animals during transport and related operations, and amending Directives 64/432 / EEC and 93/119 / EC and Regulation (EC) No 1255/97 .

Purchaser before awarding the contract, inI am calling the contractor whose bid was highest rated to be submitted within a specified time limit, not shorter than 10 days, of the following current statements or documents confirming the circumstances referred to in art. 25 paragraph 1 of the Act (Article 26 (1) of the Act), i.e.: permission) to operate in the field of running shelters for homeless animals in accordance withthe Act of 13.9.1996 on maintaining cleanliness and order in communes (i.e.: Journal of Laws of 2018, item

1454, as amended);

b) decision of the competent Poviat Veterinary Officer to grant a shelter veterinary identification number, in accordance with the Act of 11.3.2004 on the protection of animal health and combating infectious diseases (i.e. Journal of Laws of 2018, item 1967 as amended);

c) authorizede for the carrier and the certificate of approval of means of road transport

Used for long-distance transport of animals, referred to in the provisions of Council Regulation (EC) No 1/2005 of 22.12.2004 on the protection of animals during transport and related activities and amending directives 64/432 / EEC and 93/119 / EC and Regulation (EC) No 1255/97;

d) license forsheep and attendants in accordance with art. 17 clause 2 Council Regulation (EC) No 1/2005 of 22.12.2004 on the protection of animals during transport and related operations, and amending Directives 64/432 / EEC and 93/119 / EC and Regulation (EC) No 1255/97;

II. The Awarding Entity informs that pursuant to art. 24 aa of the PPL Act in the subject progressiveniu will apply the "reverse procedure"

III. The contracting authority does not provide for supplementary orders referred to in Article 67 paragraph 1 items 6 and 7

IV. In reference to Section IV.2.6) of the notice for technical reasons in order to issue the contract notice "The minimum period during which the bidder will be bound by the offer" is 2 months, with the Contractor being bound by the offer for a period of 60 days from the date setsubmitting bids.

V. The contracting authority provides for the exclusion of an economic operator based on art. 24 paragraph 5 points 1, 2, 3, 4, 5, 6, 7 and 8 of the Public Procurement Law

VI. The contractor together with the offer - a completed offer form, the recommended content of which is Annex 1 to the ToR, submits a current statement as at the date of submitting the offers, containing information that will constitute preliminary confirmation that the contractor will not beeg exclusion and meets the conditions for participation in the procedure on the form of the single European order document in the form of a "single document" (EAT). In addition to statements and documents confirming the fulfillment of the conditions for participation inproceedings indicated in the Terms of Reference and sections III.1.1) and III.1.3 of this contract notice, the Awarding Entity, before awarding the contract, will call the contractor whose tender has also been rated the highest to submit within the prescribed period, not less than 10 days, documents in the absence of grounds to exclude, including: a) [regarding national contractor] information from the KRK to the extent specified inArt. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before the submission deadline; [regarding a foreign contractor] if the contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point a), submits information from the relevant register or, in the absence of such a document, another equivalent document issued by the competent authoritycourt or administrative body of the country in which the contractor has its registered office or place of residence or place of residence has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act;

c) Statements ofa person who refuses to issue a final court judgment or a final administrative decision regarding arrears in payment of taxes, fees or social security or health insurance contributions or in the case of issuing such a judgment or decision documents confirming the payment of these claims together with any interest or fines or concluding a binding agreement regardingrepayment of these debts;

d) Statements of the contractor about the lack of a decision against him as a preventive measure forbidding the public procurement.
