The subject of the order is the delivery of preparations for washing and disinfection of transport tools and containers, accessoriow to register the process of decontamination, gas cartridges for the sterilizer.

1. The subject of the order is the delivery of:

A means for cleaning and disinfecting transport containers and carts; The agent for rinsing transport containers and carts, compatible with the detergent - disinfectantkcyjnym

2. The subject of the order is specified in detail in Annex 2.1 to the ToR.

3. The subject of the orders specified in package 1 must be admitted to trading and for use in accordance with the provisions of the Act of 20.5.2010 on medical devices (Journal of Laws of 2017, item 211, as amended) and the Law on Products bioboe (Journal of Laws from 2015 item 1926) of 9/10/2015

4. Shelf life-should be at least 18 months from the time of delivery to the Ordering Party.

1. Contractors who meet the conditions for participation in the proceedings referred to in art. 22 ust. 1b upzp regarding:

- technical or professional capacity.

In the scope of demonstrating compliance by the Contractor with the conditions referred to in art. 22 section 1b upzp must be sent before the deadline for submission-of offers - statement on the form of the Single European Order Document (hereinafter "EAT") - in accordance with the specimen of the standard form specified in the executive regulation of the European Commission issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU. b) Information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13-14 and 21 upzp, issued not earlier than 6 months before the deadline for submitting bids;
