The contract includes 1 item "Delivery of dialysate concentrates" with nomenclature units included, which are described in detail in the technical specification, part of the participation documentation. The quantities are indicative.The contracting authority is not obliged to buy the quantities specified in the specification. Participants may submit an offer for one or more nomenclatures. Classification will be done for each nomenclature.

The participants should have professional skills, namely a wholesale marketing authorization for medical devices according to the media or other equivalent document. If a participant who holds an equivalenta document, selected as a contractor, should submit a permit for wholesale trade in medical devices in accordance with the media before the conclusion of the contract. At the conclusion of the contract, the designated contractor (s) shall submit a performance guarantee of 1% of a hundredthe contract without VAT.

The guarantee may be presented in the form of a bank guarantee, a monetary sum or insurance that guarantees fulfillment through liability coverage. The form of the guarantee is at the option of the participant designated as the contractorthe performance bond shall be released upon completion of the contract and at least 30 (thirty) days after its expiration.

2. The permissible amendments to the specific contract are in accordance with Art. 116 of the Public Procurement Act

3. All participants who are not aware of the circumstances under Art. 54, para. 1, Art. 55, para. 1, items 1, 4 and 5 of the Public Procurement Law and which correspond to all previously announced by nodof the conditioner. All requirements, documents and declarations required are specified in the documentation approved by the contracting authority.

4. Requirements for the documents to be submitted at the conclusion of the contract: the documents under Art. 112, para. 1 of the PPL, as well as documents under the LMIP.

5. The bodies under Art. 47, para. 4 of the PPA are the National Revenue Agency, National Social Security Institute, PWIOS, REI, Employment Agency.

6. A participant who has the circumstances under Art. 54, para. 1, Art. 55, para. 1, items 1, 4 and 5 of the PPA, as well as the circumstances described in the documentation leading to the removal of the offer.

7. Participants can submit a bid for 1 or more nomenclatures. Ranking will be done for each nomenclature.

8. Whenthe tenderer does not submit information and / or documents additionally required by the commission appointed to conduct the procurement within the time limit specified by the tenderer;
