Reference Number: WS-Z.2380.47.2019

The subject of the order is the supply of specialized IT software in the quantity and assortment indicated in the description ofotu orders (OPZ) for a given task, constituting Annex 1 to the contract template, which is an integral part of the Terms of Reference.

The subject of the order in Task 1 is the supply of software to collect data of Facebook users in the total amount 6 pcs divided into: KWP Opole 2 pcs, KWP Katowice 2 pcs, KWP Wrocław 2 pcs, in accordance with the requirements containedin the description of the subject of the contract for this task and in the contract template

the Contractor is excluded from the procurement procedure in relation to which any of the circumstances referred to in art. 24 paragraph 1 point 12 23 PZP.

Additionally, the Employer provides for the exclusion of the Contractor pursuant to art. 24 paragraph 5 point 1 Public Procurement Law

The offer must include:

- the completed offer form for a given zrequest made using the template constituting the Annex to the ToR

- statement made on the form of the uniform European order document (EAT).

At the request of the Contractor whose bid was highest rated, submits:

1) Information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 Public Procurement Law;

2) Information from the National Criminal Record in the scope specifiedm in art. 24 paragraph 1 item 21 of the Public Procurement Law;

3) an excerpt from the relevant register or from the central record and information on business activity,

4) the contractor`s statement that he has not issued a final court judgment or the final administrative decision regarding late payment of taxes, fees or insurance premiums social or health or documents confirming payment treceivables together with any interest or fines or conclusion of a binding repayment agreement

5) declaration by the contractor that he has not been ordered to prevent public procurement;
