The subject of the order is delivery mineral aggregate for the needs of the 43rd Military Economic Department in Świętosz-w.

The subject of the order is the supply of mineral aggregatebroken fraction 0 63 mm in the amount of 12, 500 tonnes and aggregate fraction 0 31, 5 mm in the amount of 5, 500 tonnes for the needs of the 43 Military Economic Department in Świętosz-w. The subject of the order is specified in Annex 2 to the ToR (price form) and OPZ (Annex 3 to the ToR) constituting an integral part of the offer. Crushed mineral fraction 0 63 mm broken aggregate obtained as a result ofsewage of rock raw material without foreign impurities and without admixtures of clay according to PN-EN 13242 A1 and broken mineral aggregate fraction 0 31.5 mm broken aggregate obtained as a result of cutting the rock raw material without foreign impurities and without admixtures of clay according to PN-EN 13242 A1.

The Purchaser does not specify the condition of participation in the proceedings in this regard. Assessment of compliance with the participation conditionu in proceedings in this respect will be made on the basis of a declaration of compliance with the conditions for participation in the proceedings, referred to in point 7.1 of the ToR. The proceedings are conducted in electronic form via the purchasing platform, https://platformazakupowa.pl/pn/43wog Communication between the Employer and Contractors takes place only using electronic means of communication via the purchasing platform available under address https://platformazakupowa.pl/pn/43wog. Purchaser before awarding the contractcall the Contractor whose tender was highest rated to be submitted by the deadline set not shorter than 10 current days as at the date of submitting the declarations or documents specified in points 7.4 and 7.5 of the specification.

7.4. To confirm the absence of grounds for exclusion under Article 24 paragraph 1 of the Public Procurement Law and point 6.2 of the specification, the Contractor presents the following documents and statementsa, in the original in electronic form, bearing a qualified electronic signature or in the form of a copy certified as being in conformity with the original, i.e. in the form of a scan bearing a qualified electronic signature by the Contractor or an authorized person (s), in accordance with the manner of representation (at the request of the Employer):

a) information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of bids;

b) an excerpt from the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or register, in order to confirmation of the absence of grounds for exclusion under Article 24 paragraph 5 point 1 of the Act;

c) a certificate from the competent authoritya head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids or requests to participate in the proceedings, or other document confirming that the Contractor has concluded an agreement with the relevant tax authority regarding the repayment of these receivables together with any interest or fines, inin particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full of the decision of the competent authority;

d) certificate of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in default of payment of contributions for insurancesocial or health claims issued not earlier than 3 months before the expiry of the submission of tenders or requests to participate, or another document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines; in particular, he obtained a legal exemption, deferral or distributioninstallment of overdue payments or withholding in full of the decision of the competent authority.
