The subject of the contract is: Construction of a sanitary sewage system and water supply network in the area of ul. Listopadowa, Radiowa, Przytycka and Uniwersytet in Radom.

Main Site: Selected regions and streets of the city of Radom

The subject of the contract is: Construction of a sanitary sewage system and water supply system in the regon the streets of Listopadowa, Radiowa, Przytycka and University in Radom.

The material scope of the project includes:

1. construction of a PE water supply network with diameter 160 225 mm and a length of 1.13 km;

2. construction of a sanitary sewage system made of PP with a diameter of 200 mm and a length of 0.39 km.

The construction of a water supply and sanitary sewage system will be carried out in Radom in the area of ul. Listopadowa, Radiowa, Przytyckiej and University.

The Employer does not specify in this respect any requirements that the Contractor is required to demonstrate in a special way.

1. The Contractor is excluded from the contract award procedure in relation to which any of the circumstances referred to in art. 24 paragraph 1:

a) item 12;

b) item 13 lit. a c;

c) item 14, if the person referred to in this provision fromshe was convicted of an offense mentioned in art. 24 paragraph 1 point 13 lit. a c; and

d) the Contractor`s statement of the lack of a decision against him as a preventive measure forbidding public procurement. The above will take place according to the model constituting Annex 7 to IDW;

d) point 15 23 of the Act on Public Procurement Law

In addition, the Employer will exclude the Contractor pursuant to Art. 24 paragraph 5 points 1, 2, 4.

To confirm this yearthe grounds for excluding the Contractor from participating in the proceedings:

a) Information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13 lit. a c, item 14 in connection with item 13 lit. a c and point 21 of the Act, issued not earlier than 6 months before the deadline for submission of bids;

b) an excerpt from the relevant register or from the central register and information on economic activity, ifseparate provisions require an entry in the register or records in order to demonstrate the lack of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law;

c) the Contractor`s statement that he has not issued a final court judgment against him or a final administrative decision on arrears in the payment of taxes, fees or social or health insurance contributions, or in the event of such a decision being issuedtowards or decision of documents confirming the payment of these receivables together with any interest or fines, or the conclusion of a binding agreement on the repayment of these receivables. The above will take place according to the model constituting Annex 7 to IDW;

d) the Contractor`s statement that he has not been issued a preventive measure against him from applying for public contracts. Above nHowever, according to the template constituting Annex 7 to IDW.

The Employer does not specify in this respect any requirements that the Contractor is required to demonstrate in a special way. orders.

The deadline includes:

1. time to start work, i.e. 1 mone month from the date of the contract;

2. time for completion, i.e. 5 months from the date of commencement to the date of issue of the take-over certificate for the whole of the work, including obtaining the occupancy permit (if required) or effective submission of a notice of completion of the work in accordance with the law;

3. period for reporting defects, i.e. 12 months from the date of issue of the take-over certificate for the wholework robot.
