The scope of the investment task includes designing, obtaining relevant decisions and permits as well as construction of a sanitary sewerage network and a water supply network in the commune. Jabłonna, in the following scope: Construction of water supply network and sanitary sewerage network in the drainage basin of the "P6" pumping station with accompanying armament inejonie: Strażacka, Przechodnia, Piękna, Piusa XI, Żeligowskiego, Gorzyst, Słoneczna, Obrońcow Modlina in Chotomow 'and Construction of water supply network and sanitary sewerage network in the drainage basin "P7" with auxiliary equipment in the area of: Piękna, Krotka, Leśna, Jesionowa, Sosnowa, Klonowa in Chotom-w and in ul. Kolejowa, Przyrodniczej in the city of Dąbrowa Chotomowska.

Construction of a water supply network and sanitary sewage system in the P6 pumping station catchment with accompanying equipment in the vicinity of: Strażacka, Przechodnia, Pi- kna, Piusa XI, Ĺ elligowskiego, Gorzyst, SĹ, oneczna, Obrońskie Modlin in Chotom-w ', including the design and construction of: a gravity sewerage network according to the concept of approx. 5, 195 mb, a pressure sewage network according toabout 330 mb, P6 wastewater pump with a capacity of approx. 14 dm / s, water supply system according to the concept of approx. 4, 545 mb

Construction of water supply and sanitary sewerage as part of Task 5 includes municipal, poviat and railway roads as well as areas constituting private property. Piusa Street XI and ul. The partisans are poviat roads, in the area of commune roads, the following streets: Mała, Gorzyst, Żeligowskiego, Piękna-and Strażacka are public roads. Kolejowa street is located on the railway grounds constituting a closed area in accordance with Decision No. 3 of the Minister of Infrastructure and Development of 24.3.2014 on determining terrains through which railway lines run as closed terrains. who:

1. are not subject to exclusion pursuant to art. 24 sec. 1 (taking into account Article 133 paragraph 4) of the Act;

2. meet the conditions for participation in proceedings regarding: - competence or entitlements to conduct a specific professional activity, if it results from separate regulations. Determining the condition: The contracting authority does not set a specific condition in this respect,

- economic or financial situation (in accordance with point III.1.2),

- technical capacity orb (in accordance with point III.1.3)

A contractor who can not be excluded under the provisions of art. 24ust. 5 points 1 of the Public Procurement Law Act

the Employer will call the contractor whose offer has been rated the highest, to submit in a designated period, not shorter than 10 days, valid as of the date of submitting declarations or documents confirming the circumstances referred to inrt. 25 ust. 1 of the Public Procurement Law. To demonstrate the absence of grounds for exclusion from the public procurement procedure, referred to in art. 24 sec. 1 (taking into account Article 133 paragraph 4) and art. 24 sec. 5 point 1 of the Act:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline, submitsoffers or applications for admission to participation in the proceedings,

2) excerpt from the relevant register or from the central register and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 points 1 of the Act;

3) the contractor's statement that he has not issued a final court judgment against him-or a final administrative decision regarding default in the payment of taxes, fees or social security or health insurance premiums, or in the event of such a judgment or decision documenting the payment of such claims together with any interest or fines or a binding agreement on repayment of such claims; ) the contractor's statement about Braku ruling against him as a preventive measure for the prohibition of applying for public orders.

The contracting authority demands from the contractor, which relies on the ability or situation of other entities under the principles set out in Article. 22a of the Act, present for these entities the documents listed in item 6.5. 2 of the SIWZ.

The Ordering Party considers the condition to be fulfilled if: a) the Contractor demonstrates thatis insured against civil liability in the scope of activities related to the subject of the order, for a minimum guarantee sum (for Parts I and II): PLN 2, 000, 000.00

b) The Contractor will show that in a period not earlier than one month before the deadline for submission offers have financial resources or creditworthiness of at least (for Parts I and II): 1, 000, 000.00 PLN

The contracting authority will call the contractor whose offer has been rated the most, to submit in the designated, not shorter than 10 days, the current date as of the day of submitting statements or documents confirming the circumstances referred to in Article. 25 ust. 1 act, ie:

1. In order to demonstrate compliance by the contractor with the conditions of participation in the proceedings:

1) Documents confirming that the contractor is insuredBanned from civil liability in the scope of conducted activity related to the subject of the order for the guarantee sum specified by the ordering party. 2) Information of the bank or cooperative savings and credit union confirming the amount of funds held or creditworthiness of the contractor in the period not earlier than one month before the expiry of the deadline for the submission of offers.

In the case of submitting an offer for both parts by the Contractor, it is enough that the Contractor meets jointly the conditions indicated in point. 5.1. SIWZ.

List of statements or documents confirming compliance with the conditions of participation in the procedure and no grounds for exclusion are specified in item 6 of the ToR.
