a. Implementation of thermomodernization works and improvement of energy and ecological efficiency on the building of the Specialist Hospital Ludwika Rydygiera in Krak-w sp. O.o. - Long-Term Care Department inMak-w Podhalańskim (Mickiewicza 7, 34-220 Makow Podhalański St.), as part of the project "Energy modernization of voivodship public utility buildings, Measure 4.3 Improving energy efficiency in the public and residential sector, Sub-measure 4.3.3 Deep energy modernization of public buildings, regional investments, Regional Operational Program of the Wojewodztwa Małopolskiego for 2014 2020 (RPO WM) "Package 1.

3. The detailed description of the subject of the order is contained in this Specification, in particular the Functional and Functional Program constituting Annex 1A (Package 1) to the ninth Specifications, and the model agreement constituting Annex No. 3 to the Specification.

4. The Employer recommends that the Contractor should make an initial inspection of the local places of work in order toobtain information that may be necessary to prepare an offer to conclude a contract and execute the order. The Contractor will bear the costs of making a local visit.

5. Offers that do not contain the full range of the subject of the order will be rejected.

XII. INFORMATION ABOUT DOCUMENTS CONFIRMING THAT THE OFFERED PRODUCTS ARE REQUIRED BY SPECIFIC REQUIREMENTS

The Ordering Party does not request submission of documents confirming that the product offered complies with the requirements specified by the Employer.

IV. Documents are submittedon the offer: a) the offer form, according to the specimen specified in Appendix No. 2 to the Specification, - b) documents and statements confirming that the Contractor meets the conditions for participation in the Proceeding and no grounds for exclusion (mentioned in point X.1 of the Specification),

c) documents indicated in point XII Specifications (if applicable),

d) in the case of Contractors acting through a proxy fullpower, e) in the case of Contractors jointly applying for the order document stating the appointment by the Contractors jointly applying for the order of the proxy to represent them in the Proceedings for the order or representation in the Proceedings and conclusion of the public procurement contract,

f) confirmation of payment of the bid bond .

XIII. FORM OF COMMUNICATION WITHAMAWIAJĄCEGO AND CONTRACTOR

1. Communication between the ordering party and the contractors takes place between the Ordering Party and the Contractors is done using the miniPortal https://miniportal.uzp.gov.pl/, ePUAP https://epuap.gov.pl/wps/portal and e-mail.

2. Electronic documents, statements or electronic copies of documents or declarations are submitted by the Contractor via the Formfor communication as attachments. The Employer also allows the possibility of submitting electronic documents, statements or electronic copies of documents or statements by e-mail, to the e-mail address given in point 6.

I. ingeneral formations

1. The Contractor intending to take part in the public procurement procedure must have an ePUAP account. The contractor holding an account on ePUAP has access to the following forms: submission, modification, withdrawal of the offer or application and to the form for communication.

2. Technical and organizational requirements for sending and receiving electronic documents, electronic copies of dokumentow and statements and information provided using them are described in the Regulations for the use of miniPortal and the ePUAP Regulations.

5. The identifier of the proceedings and the public key for a given procurement procedure is available on the list of all proceedings on the miniPortal and constitutes an attachment to this SIWZ.

II. Submission of offer

1. The Contractor shall submit an offer for indirectand the form to submit, change, withdraw an offer or application available on ePUAP and also made available on the miniPortal. The public key necessary to encode the offer by the Contractor is available to the Contractors on the miniPortal. In the offer form, the Contractor is obliged to provide the mailbox address (e-mail address) with which correspondence related to the proceedings will be keptIEM.

2. The offer should be prepared in Polish, while maintaining the electronic form in the .doc, docx, .rtf, .pdf data format and signed with a qualified electronic signature. The method of submitting the offer, including the encryption of the offer, is described in the Regulations for the use of miniPortal. The offer must be submitted in the original.

7. Form of submitting documents:

a. Documents or statements about whichin X.2), XI, [XII) if applicable] SIWZ, are submitted in the original in the form of an electronic document or in an electronic copy of the document or a statement certified as a true copy of the original.
