The subject of the order is:

Main Site: Łodź, ul. Revolution 29

Implementation of the task entitled: Reconstruction of existing buildings within the boundaries of plots at ul. Revolution 29 in Ł-dź, including land development and the necessary technical infrastructure based on the Construction and Executive Project.

Investment taskis based on the revitalization of the real property located at ul. Revolution 29 in Ł-dź. The order will include: execution of construction works related to the reconstruction of existing buildings along with land development within the boundaries of the plot no. 417/1, precinct S-1. The investment is an element of Project 1 of the District Revitalization Center of Ł-dź. As a result of the revitalization, the development of the property at ul. Rewolucji 29 will maintain a residential function with retail and service premises on the ground floor accessible directly from the street. The outbuildings (with entrances from the side of the courtyard) have been entirely devoted to residential functions. The expected result of revitalization is the restoration of the material and functional value of the development and land development of the plot, including elements of small architecture, urban infrastructure (axislighting, monitoring), reclamation and increasing the share of biologically active area in the overall land balance. The real property, which due to the degree of technical wear has been qualified to the degraded area, will regain functional, aesthetic and high technical standards. Thanks to the revitalization of the city's resources, there will be municipal housing unitsapplicable technical conditions for residential areas, which will have a positive impact on the quality of life of the inhabitants of Środszaście Łodzi. Reconstruction of outdated installation systems, especially heating installations generating pollutants will reduce harmful emissions to the environment.

The main objective of the investment is to obtain a revitalized real estate. The main task in the circlees for the transformation of the functional-spatial arrangement of the residential part is the reorganization of the residential area.

The detailed description of the order is included in Volume III of the Terms of Reference of the Subject of the Order and the Functional Program with attachments (for part I) and Volume II of the Contract Act (for parts I).

The ordering party does not set a specific condition in this forthe limit.

1. Documents confirming fulfillment of the conditions for participation in the proceedings and no grounds for exclusion, which based on art. 26 par. 1 of the Act will be required to submit the contractor whose offer was rated the highest - at the request of the Employer within the prescribed period of not shorter than 10 days, valid as of the date of their submission.

2. In order to confirm fulfillment of the conditions of participation in the procedure. Orderrequesting the following documents (Article 25 paragraph 1 point 1 of the Act): - information of the bank or cooperative savings and credit union confirming the amount of funds held or the creditworthiness of the contractor, in a period not earlier than one month before the deadline for submission of bids. If, for a justified reason, the contractor can not submit the documents required by the contractorntow, referred to above, the Employer allows the contractor to submit other documents referred to in Article 26 (2c) of the Act

1. Contractors, who prove: no grounds for exclusion from the proceedings under Art. 24 sec. 1 (obligatory grounds for exclusion) and art. 24 paragraph 5 of the Act (optional grounds for exclusion) and meet, on the levelNot described below conditions for participation in the proceedings, referred to in art. 22 ust. 1 of the Act

2. Due to the limited number of marks in Section III.1.2) and III.1.3), the selection criteria, as well as statements and documents to be provided to the contractor, confirming compliance with the conditions for participation in the proceedings and demonstrating the absence of grounds for exclusion have been specified in the order documentsVolume I SIWZ Instructions for Economic Operators posted on the website of the contracting party. Candidates' information on the conditions of participation and grounds for exclusion and a list of statements or documents confirming the fulfillment of conditions for participation in the proceedings and no grounds for exclusion can be found in Sections 6 and 7 of the ToR.

3. A statement constituting a preliminary confirmation of compliance with the conditions of participation in the postand the lack of grounds for excluding the contractor from the procedure - in the form of a single European order document (the so-called EAT). A statement made in accordance with the provisions of point 8 SIWZ. The Employer submits in the form of a separate .xml file, which after importing should be completed, saved, signed and forwarded by the Contractor in an electronic version (plik.pdf) to the Ordering Partyto the address: zamowienia@zim.uml.lodz.pl within the deadline for submitting offers. (Detailed instructions - see: item 7.1.1., Item 8, items 11.10, 12.3 of the ToR).

4. At the request of the Ordering Party, in order to confirm the absence of grounds for exclusion of the Contractor from participation in the proceedings, the Contractor is obliged to submit information, certificates, excerpt and statements referred to in - 5 of the Regulation of the Minister of Development dated26.7.2016 on the types of documents that the Employer may request from the contractor in the proceedings for the award of the contract (Journal of Laws of 2016, item 116)

5. If the Contractor is established or resides outside the territory of the Republic of Poland, and also in the case of a person whose document concerned is domiciled outside the territory of the Republic of Poland, the rules ofeled in - 7 and 8 above Regulation.

6. The ordering party, in accordance with art. 24aa paragraph 1 of the Public Procurement Law, provides for the possibility of firstly assessing the offers, and then investigating whether the Contractor whose tender has been rated as the most advantageous, is not subject to exclusion and meets the conditions for participation in the procedure. The offer binding date is -90 days

8. Deadline-Implementation of the Investment:

For Part I:

The Completion Date of the Investment is set by the Parties for 16 months from the date of conclusion of the Contract.

For Part II:

The Completion Date for the Investment is set by the Parties for 13 months from the date of conclusion of the Agreement. The Contractor, within 3 days from publishing the information on the website, from the opening of offers, provides the orderer with a declaration of affiliation-or the absence of affiliation to the same group, referred to in art. 24 sec. 1 point 23 of the Act
