Reference Number: A-AZ-22-1723 / 18 - The subject of the order is the delivery of computer hardware and software for the National Institute of Public Health Of the National Institute of Hygiene as part of the ProfiBaza project, accnot with OPZ (Annex 1 to SIWZ, hereinafter: "OPZ"), containing the detailed specifications of the ordered computer equipment and software.

The subject of the order is the delivery of computer hardware and software for the National Institute of Public Health of the State InsHygiene titles as part of the ProfiBaza project, in accordance with OPZ (Annex 1 to SIWZ, hereinafter: "OPZ"), containing the detailed specifications of the ordered hardware and software, including requirements for the needs ..

Contractors may apply for the contract, who meet the conditions for participation in the proceedings - Concerning the economic situation and technical or professional capacityspecified in items III.1.2 and III.1.3. Notices about the order and who are not subject to exclusion from the proceedings pursuant to art. 24 sec. 1 of the PZP Act and art. 24 section 5 point 1 and 8 of the Public Procurement Law Act

1) In confirmation of the lack of grounds for exclusion, the Contractor shall submit together with the offer a declaration in the form of a Single European Order Document (EED) prepared according to the form (form) constituting-Annex No. 3 to the ToR, concerning: a) contractors (in the case of contractors jointly applying for the ordering of each of them) with regard to the lack of grounds for exclusion (part II, III to the extent specified in Article 24 paragraph 1 and section 5 point 1 and 8 of the Public Procurement Law Acts

b) entities for which the contractor's resources are set up to demonstrate compliance with the conditions of participation in postingin the scope of the absence of the grounds for exclusion (Part II, Part III, respectively, to the extent specified in Article 24 paragraph 1 and section 5 point 1 and 8 of the Public Procurement Law,

2) In order to determine whether there is a ground for excluding the Contractor from Proceedings under art. 24 sec. 1 point 23 Pzp, the Contractor, within 3 days from posting information on the website of the Employer-which is mentioned in art. 86 par. 5 of the PPL, shall provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the PPL on the form constituting Annex No. 5a to the ToR. In the case of being part of the same capital group, the Contractor together with the submission of a statement may provide evidence that links with another contractorlead to distortion of competition in the Procurement Procedure

3) In confirmation of the lack of grounds for exclusion, the Contractor whose tender will be considered the most advantageous will be requested to submit the documents indicated in clause 7.1.9.3 of the SETC - Confirmation of no grounds for exclusion of the Contractor or entity, the potential of which the Contractor relies to meet the conditions of the contractin the proceedings.

4) The declarations are submitted in the original. Documents must be submitted in the form of an original or a copy certified as "true to the original."

The Contracting Authority will consider that the Contractor meets the condition of participating in the Proceedings in the economic and financial situation, if the contractor has financial resources or creditworthiness in an amount not less than 500, 000 PLN. The contractor maythat in order to confirm compliance with the conditions of participation in the Procedure, rely on the financial situation of other entities, regardless of the legal nature of the legal relationships that it has with it, on the terms set out in Art. 22a Pzp.

1. Each contractor may submit only one offer as part of this proceeding.

2. The offer includes:

- completed form (s) EAT,

- proof that realizing fromAmending the Contractor will have the necessary resources of other entities when it relies on their abilities or situations (eg liabilities -

Statements), referred to in points III.1.2 or III.1.3 of this announcement (if applicable),

- completed Bid Form ( according to Annex No. 2 to the ToR), - authorization to represent Contractors jointly applying for the award of the contractnot with the provisions of point 8.2. and 8.3 SIWZ (if applicable),

- document confirming the right to sign the offer, eg a registration document and, if applicable, a power of attorney to sign the offer (in case when the authorization to sign the offer does not result directly from the registration document); pay the bid bond, in accordance with the provisions of point 15 of the ToR, - evidence about whichch referred to in art. 24 sec. 8 of the PPL Act, that despite the grounds for exclusion listed in art. 24 sec. 1 item 13 and 14 and 16 20 and par. 5 points 1 and 8 of the PZP Act, the measures taken by it are sufficient to demonstrate its reliability (if applicable);

3. The deadline for the implementation of the contract will start on the day it is signed.

4. The Contracting Authority informs that this order is co-financed frommiddle of the ProfiBaza Project co-financed from the EU centers under the POPC on the basis of a co-financing agreement. In connection with the above, the Ordering Party reserves the right to refuse to accept the subject of the order at the stage of:

- final acceptance if the subject of the Agreement will not be performed within the deadline specified in this announcement as the date of order completion. ... will benefit from the right to refuseif you do not get permission to extend the period of eligibility of expenditure in the contract for co-financing.

5. The Contracting Party reserves that according to art. 93 para. 1a of the PPL Act. may invalidate the procurement procedure if the funds from the European Union budget and non-recoverable funds from the assistance provided by Member States of the European AgreementFree Trade Agreement (EFTA), which the contracting authority intended to allocate to finance all or part of the contract, will not be awarded.
