1. The subject of the order is the delivery of computer hardware and software and the development of teleinformation networks in SPZOZ Bychawa. The subject of the orders has been divided into 3 Parts:

PartsNo. 1:

- Purchase of the ICT network performance service in accordance with the project,

- Network devices including:

• 10 x 10Gb switch - 1 pcs,

• POE switches -220-24p-GE2 -3 pcs;

- UTM subscription for updates - 1 item,

- UTM anti-virus software for workstations - 125 items,

- Access Point cooperating with the controller - 18 items,

- WIFI network controller - 1 item,

- Management systemNetwork - 1 pcs,

- UPS 1000 VA uninterruptible power supply, LCD, for cabinet, 2U 230 V - delivery and assembly - 3 pcs. • Part No. 2:

- Database server - delivery and configuration, operating system - 2 pcs,

- RACK server cabinet - delivery and assembly - 1 item,

- Application server (witrulization system) - delivery and configuration, virtualization, operating system - 1pc,

- UPS uninterruptible power supply - 1 pcs,

- Computer setse - purchase, assembly and configuration - 125 items,

- Matrix for database and application servers - 1 pc,

- NAS 4x4TB -1 pcs. - Part No. 3:

- IT system - position with EDM - Clinic Doctor - 18 items,

- Integrated Information System - update of the position with the EDM Module - 95 items,

2. The detailed list of elements which are the subject of the order is included in Attachment No. 2 to the ToR bid form.

3. Detailed information about the requirements. Employersit regarding:

a) description of the subject of the order,

b) technical parameters of computer hardware, matrix servers and software

They are described in Appendix No. 1 to the ToR. Description of the subject orders / Table of technical parameters.

The deposit for the entire subject of the order is set at 15 PLN 600.00 (say: fifteen thousand six hundred zlotys 00/100).

The contractor is obliged to pay wadium in the amount of the sum of amounts corresponding to the parts to which the proceeding proceeds:

3 - 3200, 00

2. The bid bond may be contributed in one or more of the following forms, referred to in art. 45 par. 6 Pzp

The contractors who are entitled to the award of this order are:

1) They are not subject to exclusion,

2) They meet the conditions of participation-in proceedings regarding: - a) Competence or entitlements to conduct a specific professional activity, if it results from separate regulations - the Employer does not set specific requirements in this respect.

b) Economic or financial situation.

The ordering party does not set specific requirements in terms of of this condition.

c) Technical or professional capacity - detailed description in section III.2.3) Kwtechnical aliquotations

2. Potential of the third party:

1) The Contractor, in order to confirm compliance with the conditions for participation in the proceedings in relevant situations and in relation to a specific order, or a part thereof may rely on the technical, professional or financial or economic capacity of other entities, regardless of the legal nature of the merger with him the relationship of rights2) the Employer informs that "the relevant situation referred to in point 1) will occur only when:

a) The Contractor, which relies on the abilities or situation of other entities, will prove to the Ordering Party that while performing the order, he will have the necessary resources of these entities, in particular by presenting the obligation of these entities to give them the necessary resourcesfor the needs of the order (including at least the information given in point (c)),

b) the Employer will assess whether the professional capacity of third parties made available to the Contractor or their financial or economic situation allowing the Contractor to demonstrate compliance with the conditions for participation in the procedure and whether there is any ground of exclusion regarding this subject, referred to in-Art. 24 section 1 items 13-22 and section 5 points 1 and 8 of the Public Procurement Law;

c) The obligation or other documents confirming the availability of resources by other entities must clearly and unambiguously result in particular: - scope of resources of another entity available to the Contractor, by the Contractor, when executing the order,

- scope and period of participation of another entity when executing the order.

3. Contractors may onfield apply for the order. In this case, the Contractors shall appoint a Proxy (Leader) to represent them in these proceedings or represent them in the proceedings and conclude a public procurement contract. All correspondence will be carried out only with the Proxy (Leader). Powers of attorney in writing (the original or a copy confirmed for compliancewith the original by a notary public) must be attached to the offer. The power of attorney referred to in the preceding sentence should: - - specify the scope of the authorization, - replace all Contractors who jointly apply for the order, - each of these Contractors must sign a power of attorney or other document drawn up for this purpose on a common document; alternatively - issue nIndependent power of attorney for the Plenipotentiary - Leader (signature of the Plenipotentiary - Leader on the power of attorney document is not required).

4. The provisions concerning the Contractor shall apply accordingly to Contractors jointly applying for the order.

5. In the case of Contractors jointly applying for the award of a contract, the condition set out in point 1.2 (c) must meet at leastej one Contractor alone.C.D. in section III.2.2)
