The purpose of this release is to enter into a non-exclusive agreement, e.g. Purpose of v. Prevention services i. S. of -5 SGB XI in the context of a so-called. Open-house model

1) The basis of the "Optimal food selection" qualification module are:

The nursing care funds of the AOK in Hessen u. The BAHN-BKK intend to conclude non-exclusive agreements with suitable companies for the purpose of promoting prevention measures within the meaning of the provision of - 5 SGB XI. The companymen carry out:

- Qualification measures on the subject of "Nutrition - Optimal Food Selection" in the partially and fully inpatient care facilities,

- hand out a so-called handout and a certificate of attendance to the participants,

- carry out the quality assurance after the implementation on the basis of a feedback sheet,

- the overall evaluation of the feedback sheets will be made to the AOK nursing care fundin Hessen.

It should be noted that the expenses of the long-term care insurance funds for performing their tasks for the purpose of promoting prevention measures in accordance with Section 5 (2) SGB XI totaled 0.30 for each of their insured persons in 2016 Amount to EUR. The expenditure in the following years is in accordance with the percentage change in the monthly reference sizeaccording to - 18 paragraph 1 of the fourth book. The reference size is determined by the Schaetzerkreis and announced annually by the Federal Ministry of Health in September / October. Therefore, the amount of expenditure (financial resources that may be used in this connection for the purpose of promoting prevention measures) is in accordance with the regulation of - 5 paragraph2 SGB XI limited by law. The care fund of the AOK in Hesse is on behalf of the care funds, to monitor the amount of financial expenditure legally stipulated for prevention according to - 5 Para. 2 SGB XI and exclusively for the promotion of the tasks stipulated legally in - 5 SGB XI use. The AOK nursing care fund in Hesse has a contRolling mechanism set up in your house.

The financial support ends upon written notification by the nursing care fund of the AOK in Hesse with immediate effect if it is determined in the context of the controlling to be carried out that the available and legally restricted financial resources could be exceeded or were consumed. The provider directory is made available to the care facilities by the care fund of the AOK in Hesse. The companies agree that their data will be included in the aforementioned directory and made available to the care facilities in Hesse after the contract has been concludedbe put. The provider himself has no right to be considered when carrying out the qualification measure. As a result, the provider may have the underlying contract, but it may not be used once in the course of the contract; therefore in this case also has no right to remuneration.

A contract is concluded with every company that fulfills the requirements for participation and documents this by submitting the evidence / signing of the required declarations mentioned under III.1.1), III.1.2) and III.1.3 and the agreement itself. There is no exclusivity.

The earliest possible start of the contract or accession isthe 1.1.2020. The contract ends on December 31, 2021. The companies can join this contract until July 1, 2021 on the same terms, provided that they submit the contract documents including the associated attachments no later than April 30, 2021 to the parties listed under I.1. have requested the specified contact address and this will be sent in full by May 30, 2021 to the address listed under I.1. sign the named contact pointt submitted.
