"Purchase and delivery of an initial electric-shredder"

1. The subject matter of the order is the purchase and delivery of an initial electric shredder.

Main Site: Zakład Zagospodarowania Odpadow MZC Sp. oo, 22-200 Włodawa, POLAND, div.an no .: 47 / 2.

1. Brand new low-speed mobile electric shredder with a split functionbag sackers for mixed municipal waste and a grinder for large-size waste, with a maximum main engine power, at least 130 kW, not more than 250 kW.

2. The device can not be a prototype and should come from serial production, it should be CE marked.

3. Operational efficiency min. 15 Mg / h for mixed municipal waste and min. 5, 0 Mg / h for large-size wastethese.

4. Mobile device mounted on a frame for transport on a hooklift (in accordance with the applicable standards), with an auxiliary, hydraulically extended chassis with pneumatic wheels, enabling displacement through, for example, a wheel loader.

5. Weight of the device ready for work: 20-25 Mg.

6. Grinding chamber:

1) equipped with one or two shredding shafts,

2) for one weighbridgefurnace equipped with an averageing comb, in the case of two shredding rollers equipped with a removable breaking beam, installed between shafts,

3) loading height: max. 3, 000 mm,

4) loading width: 2, 300 mm 5, 000 mm,

5) shaft length / about grinding:

a. min. 3, 000 mm for single-shaft machines,

b. min. 1 700 mm for twin shaft machines,

6) armored shaft shafts or shafts,

7)-shaft rotational speed / w during operation no more than 37 rpm., 8) possibility of automatic reversing of shaft operation during operation under load.

7. Readable control panel, containing at least indicators:

1) number of hours worked since the beginning of operation,

2) number of hours worked per day,

3) rotational speed of shaft / -for shredding,

4) information-about errors and malfunctions,

5) selected program (automatic machine start according to the user's parameters such as shaft speed / ow, shaft reversal program / ow).

8. Spout conveyor:

1) tape width: min. 1, 000 mm,

2) the length of the conveyor should be at least 5, 500 mm,

3) the type of feeder tape: with the transverse zebras to the axis of the feeder,

4) the rear conveyor, folded and splitgiven hydraulically.

9. Drive assembly built-in.

10. It is required that the device be mobile, adapted to work in the open space, resistant to weather conditions.

11. The device is equipped with a control cabinet installed inside the body of the machine (constituting its integral element).

12. The crusher should be equipped with an industrial connection socket suitable for power forinstalled. The socket should be located on the machine body in a place remote from the loading area. Additionally, as part of the order, the Contractor will provide two plugs compatible with the connection socket on the machine and a power cable (at least 20 m for each plug).

13. The device should be equipped with emergency stops at critical points of the machine in accordance with knowledge and experienceby the producer.

14. The device should be equipped with a system that turns the device off when it is overloaded.

15. Separator of ferrous metal metals overhead with a chute, hydraulically adjustable height.

Ordering in accordance with point. 7.5. point. d) The ToR, before awarding the contract, will call the contractor whose offer will be the highest rated to submit in the designated, not shorter than 10 days, including current as of the day of submitting the excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Public Procurement Law - List of statements or documents confirming compliance with the conditions of participation in the proceedingsand no grounds for exclusion:

1. The offer will be accompanied by the Contractor by the current statement on the day of the submission of offers in the form of an EAT, in accordance with the model constituting Annex no. No. 3 to Part I of the ToR in the scope indicated by the Contracting Party in this ToR. The information contained in the statement is a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings. In this case, the Contractor does not have to complete any of the other sections of part IV of the qualification criteria form, and the relevant (evidence-based) verification of compliance with specific conditions of participation in the procedure (selection criteria) specified by the Employer, the Employer will make based on relevant documents submitted by the Contractor, whose offer was evaluated at the highest, at the request of the Employer (Article 26 paragraph 1 of the PPL). 3) With the completed EAT form, follow the instructions for submitting the Single European Order Document using electronic means of communication, available at the following website: https://www.uzp.gov.pl/__data/assets/pdf_file/0025/36196/Instrrukcja -skladania-go-electronic.pdf 2. The contractor who refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and compliance with them, in so far as it relies on their resources, the conditions for participation in the proceedings also include uniform documents (EAT) concerning entities together with the power of attorney to sign it. 3. In the event of a joint application by the contractorin, EAT and power of attorney to sign it, each of the Contractors jointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion to the extent that each of the Contractors demonstrates fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion. 4.With the offer, the Contractor is obliged to submit: 1) Current onń submission of offers - a uniform European order document (EAT) submitted in electronic form to the address of the Ordering Party: sekretariat@zzowlodawa.pl, constituting, as an example, Annex No. 3 to Part I of the ToR for the purpose of preliminary confirmation that the Contractor meets the conditions for participation in the proceedings and is not subject to exclusion. 2) Tender offer, constituting Annex No. 1 to part I of the Terms of Reference together with the technical offerwhich constitutes Annex No. 2 to Part I of the ToR. 3) Proof of payment of the bid bond. 4) Obligation or other documents of entities to give the Contractor at the disposal of the necessary resources for the needs of order fulfillment, if the Contractor relies on the capabilities of other entities. 5) In the event of the Contractors jointly applying for the order, the offer must be accompanied by an establishing documentproxy to represent them in the proceedings for the award or representation in the proceedings and conclusion of the public procurement contract in accordance with art. 23 sec. 1 and 2 of the Public Procurement Law, if it does not result from the JEDZ attached to the offer. 6) If the offer is signed by the attorney on behalf of the Contractor, the offer should be accompanied by a power of attorney specifying its scope and signedby persons authorized to represent the Contractor, if it does not result from the JEDZ attached to the offer. 7) if the EAT on behalf of the Contractor is signed by the proxy for the offer, attach a power of attorney defining its scope and signed by persons authorized to represent the contractor.
