Production and delivery of 960 pieces of AT overalls from flame retardant fabric.

The subject of the order has been described in detaily in Annex 4 to this SIWZ. - A Contractor who meets the conditions for participation in the procedure may apply for the order and is not subject to exclusion from the proceedings under Art. 24 sec. 1 and 5 of the Public Procurement Law. In order to demonstrate the lack of grounds for exclusion from the procurement procedure, the Contracting Authority will request documents: - - a copy from the relevant register or from central records and informationof economic activity, if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act, - a certificate from the competent head of the tax office confirming that the contractor is not in arrears with paying taxes, issued not earlier than 3 months before the deadline for submission of bids or applicationincluding admission to participate in the proceedings, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or installment payment of overdue payments or withholding in full implementation of the decision of the competent authority,

- zconfirmation of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids or requests for participation in the proceedings, lor another document confirming that the contractor has entered into an agreement with the competent authority regarding the repayment of such receivables with any interest or fines, in particular it obtained the legally required exemption, deferral or payment in installments of overdue payments or suspension in full of the competent body's decision,

- information from the National Criminal Register in the scope ofin art. 24 sec. 1 items 13, 14 and 21 of the Act and, regarding convictions for a prison sentence, to the extent specified by the ordering party pursuant to art. 24 sec. 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting offers or applications for admission to participate in the proceedings, - statements of the contractor about the lack of a final court judgment against himu or a final administrative decision on default of taxes, fees or social security or health insurance premiums or - in the event of such a verdict or decision - documents confirming the payment of such claims together with any interest or fines or a binding agreement regarding repayment of such receivables,

- statements by the contractor about Br- a statement by the contractor about the lack of a final conviction of a conviction court for misdemeanor of a restriction of liberty or a fine to the extent specified by the awarding entity on the basis of art. 24 sec. 5 points 5 and 6 of the Act, - statements by the contractor about the lack of a final decision against him aof the obligation to comply with the provisions of labor law, environmental protection law or social security provisions in the scope specified by the awarding entity pursuant to art. 24 sec. 5 point 7 of the Act,

- the contractor's statement on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws from 2016th item 716) - the Contractor having its registered office or place of residence outside the territory of the Republic of Poland submits documents in accordance with the description contained in para. 6 chapter VII of the SIWZ. The terms of participation and required documents are specified in chapter VI and VII of the ToR. The contracting authority provides for conducting proceedings in the so-called reversed procedure, referred to in art. 24aa paragraph 1 of the Public Procurement Law
