The subject of the order is: package No. 1 comprehensive cleaningthe premises of the Provincial Integrated Hospital named after dr. Roman Ostrzycki in Konin at ul. Card. S. Wyszyńskiego 1 along with the distribution of meals and washing dishes, internal transport of patients, transport of corpses from hospital wards to the storage place, i.e. the cold store in the Pathomorphology Department. A detailed description of the subject of the contract is included in Annex 3 and 3.1 to 3.30 toTerms of Reference. Package No. 2 comprehensive cleanliness in the rooms of the Wojewodzki Combined Hospital named after. dr. Roman Ostrzycki in Konin at ul. Szpitalna 45 along with the distribution of meals and washing dishes, internal transport of patients, transport of corpses from hospital wards to the storage place, i.e. the cold store in the Pathomorphology Department. A detailed description of the subject of the order is includedrty is in Annex 4 and 4.1 to 4.31 to the ToR.

The object of the contract is:

- package No. 1.

The subject of the contract includes comprehensive cleaning at the premises of the Provincial Integrated Hospital named after. dr. Roman Ostrzycki in Konin at ul. Card. S. Wyszyńskiego 1 along with the distribution of meals and washing dishes, internal transport of patients, transport of corpses from hospital departments to the storage place, i.e. the cold store in the Pathomorphology Department. A detailed description of the subject of the contract is included in Annex 3 and 3.1 to 3.30 to the ToR, which the Employer will make available on the website (www.szpital-konin.pl) and (platformazakupowa.pl).

Zamthe aviator provides for the award of a similar contract in accordance with art. 67 section 1 point 6. The subject of the order will be services falling within the scope of the basic order with a value not exceeding 40% of the basic order. The contract will be awarded on the free hand basis after negotiations have been conducted, the subject of which will be the price and other provisions of the contract regarding the performance of the contract, between andOther: possibilities of changing the size of the total area for cleaning and disinfection, the number of departments (blocks, factories), outages of some departments, changes in the number of m between individual zones.

The Employer does not describe the condition regarding the ability to conduct professional activity, including the requirements related to entry in the professional or trade register.

8.4. In order to be confirmedand the lack of grounds for excluding the Contractor from participating in the procedure, the Employer will request, in accordance with point 8.2 of the ToR, the following documents:

1) information from the National Criminal Register in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for submission of bids;

2) an excerpt from the relevant register or from the central register and information oneconomic viability, if separate provisions require an entry in the register or records to confirm the absence of grounds for exclusion under Article 24 paragraph 5 point 1 of the Act;

3) the Contractor`s statement that he has not issued a final court judgment against him or a final administrative decision on late payment of taxes, fees or social or health insurance contributions, orin case of issuing such a judgment or decision of documents confirming the payment of these debts together with any interest or fines or concluding a binding agreement on the repayment of those debts;

4) the Contractor`s statement that he has not been ruled against it as a precautionary measure against the prohibition of competing for public contracts;

5) the Contractor`s declaration of belongingand or not belonging to the same capital group; in the case of belonging to the same capital group, the Contractor may submit, together with a statement, documents or information confirming that links with another Contractor do not lead to distortion of competition in the proceedings.

8.5. Contractor having its registered office or place of residence outside the territory of the Republic of Poland.

1) If Wthe contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in:

a) point 8.4 point 1) submits information from the relevant register or, in the absence of such register, another equivalent document issued by a competent judicial or administrative authority the country in which the Contractor has its registered office or place of residence or place of residencehas a person whose information or document relates to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act. This document should be issued not earlier than 6 months before the deadline for submission of bids;

b) point 8.4 item 2) submits the document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming accordingly that its liquor has not been openedidation or bankruptcy. This document should be issued not earlier than 6 months before the deadline for submission of bids.

2) If in the country where the Contractor has its registered office or place of residence or place of residence there is a person to whom the document relates, no documents referred to in point 1), they are replaced by a document containing the relevant Declaration of Executeof the general public, indicating the person or persons authorized to represent it, or the statement of the person to whom the document was to be submitted, lodged before a notary public or before a judicial, administrative or professional or economic self-government body competent for the seat or place of residence of the Contractor or the place of residence of that person . This document should be issued not earlierless than 6 months before the deadline for submission of bids.

The Employer does not describe the condition regarding the economic situation and financial.

12.1. The Contractor is required to provide a deposit of:

-Package No. 1 68 750.00 PLN,

- Package No. 2 59 160.00 PLN.

12.2. The bid bond must be lodged before the submission deadline in one or more of the following forms, depending on the choice of the Contractor:

1) in cash, by transfer to a bank account: 92 1020 4027 0000 1102 1312 6414 by 10.00;

2) bank guarantees or cooperative sureties from a savings and credit union, includinge guaranteeing the cash register is always a monetary guarantee;

3) bank guarantees;

4) insurance guarantees;

5) guarantees granted by entities referred to in art. 6b paragraph 5 point 2 of the Act of November 9, 2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2014, item 1804 and of 2015, items 978 and 1240).

12.3. The bid bond paid in the form of sureties or guarantees:

1) The Contractor shall submit in electronic form by placing, in accordance with point 10, the original bid bond document, i.e. with a qualified electronic signature of the persons authorized to issue it, i.e. the issuer of the document;

2) must cover the entire period of the offer;

3) should be issued to the Employer;

4) it is necessary that the warranty or guarantee covers responsesFor all cases resulting in the loss of the bid bond by Wyk, specify in art. 46 section 4a and 5 section

8.9. Documents regarding third parties and subcontractors.

1. In order to assess whether the Contractor relying on the abilities or situation of other entities on the principles set out in Art. 22a of the Act, will have the necessary resources to the extentenabling proper performance of the public contract and assessing whether the relationship between the Contractor and these entities guarantees real access to their resources, the contracting authority requests documents that specify:

1) the scope of the Contractor`s resources of another entity;

2) the use of the resources of another entity by the Contractor, when performing a public contract;

3) scope and period of participationanother entity in the performance of the public contract;

4) whether the entity on whose abilities the Contractor relies in relation to the conditions of participation in the proceedings regarding education, professional qualifications or experience, will carry out the works or services to which the abovementioned abilities relate.

2. The Employer requests from the Contractor, which relies on the abilities or situation of other entitiesin accordance with the principles specified in art. 22a of the Act, present in relation to these entities the documents listed in point 8.4 item 1 4 of the Terms of Reference.

3. The Employer requires the Contractor to submit the documents listed in point 8.4 item 1 4 of the Terms of Reference regarding the subcontractor to whom he intends to entrust the performance of part of the contract, who is not the entity on whose abilities or situation the Contractor reliesand on the principles set out in art. 22a of the Act.

C.d. documents of Contractors having their registered office or place of residence outside the territory of the Republic of Poland:

8.6. The Contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whom the document referred to in point 8.4 point 1) applies, submits the document referred to in point 8.5 point 1), to the extent specified in art. 24 paragraph 1 item 14 and 21 of the Act. If in the country in which the person to whom the document was supposed to reside does not seem such documents, it is replaced by a document containing a statement of that person submitted to a notary public or before a judicial, administrative or professional or economic self-government body competent for the placese that person`s residence. This document should be issued not earlier than 6 months before the deadline for submitting bids.

8.8. In order to confirm that the services offered meet the requirements specified by the Employer, the EmployerIn accordance with point 8.2 of the ToR, the applicant will demand the following documents:

For packages 1 and 2:

a) procedure or instructions for washing and disinfecting vessels for excretions and secretions;

b) procedure or instructions for the preparation of the working solution of the disinfectant (for each agent indicated in the requirements for disinfectants);

c) the procedure or instructions for washing and disinfecting hands;

d) proceduresę or instructions for cleaning and disinfecting dispensers for hand, soap and paper disinfectant, including the replacement label with each supplement, containing the following information: name of the preparation, series, expiry date, date of completion, supplementary signature;

e) procedure or instructions for providing the service in ward ovens;

f) fasting procedure or instructionhandling equipment used for cleaning;

g) procedure or instruction for handling dirty and clean mops;

h) procedure or instruction for post-exposure procedure for blood and other potentially infectious biological material, including notification of the Epidemiological Specialist;

i) procedure or instruction corpse transport;

j) list of cleaning agents and required disinfectantsaccording to the description in the attachment to the ToR, requirements for disinfectants;

and for package No. 1:

1) hygiene plan based on the list of washing and preserving and disinfecting agents, which were taken into account when preparing offers depending on the place of use and type dirt, including zoning, equipment and products, as well as cleaning techniques from stitchesshagging of the hygiene plan for the gynecological operating room, cutting rooms, delivery rooms, neonatal department, lung and tuberculosis department and cytostatic drug laboratory.

For package No. 2:

2) hygiene plan based on the list of cleaning and preserving and disinfecting agents, which were taken into account when preparing offers depending on the place of use and type of soilingincluding zoning, equipment and measures, and cleaning techniques detailing the hygiene plan for the operating theater, anesthesia and intensive care unit, observation and infectious ward with a children`s infectious unit and a sterile recipe laboratory.
