Maintenance of cleanliness in the property of the District Court in Gdańsk

The subject of the order is to maintain cleanliness in the real estate of the District Court in Gdańsk in two separate locations. This order is divided into two parts: part I to keep clean at facilities in Gdańsk, part II to keep clean at the facility in Sopot

Main Site: Gdańsk

Keeping the facilities in Gdańsk at 3 Maja 9A street; cleaning inside, outside, exchange of medias in toilets, window cleaning, in addition to the person on duty during the Court's working hours and in the building at ul. Nowe Ogrody 30/34, cleaning inside, window cleaning, additional person on duty during the Court's working hours.

1. The Employer does not formulate the condition of participation in this area.

Nin accordance with the content of the SIWZ, contractors may apply for the award of the contract, who will demonstrate the lack of grounds for exclusion from the proceedings based on the premises indicated in Article 24 para. 1 PPA and 24 para. 5 Pzp, indicated in the content of the ToR. In confirmation of the lack of grounds for exclusion, the contractor submits:

a) Declaration of EED - along with the offer.

B) Information about membership or non-membershipto the same capital group - according to appendix to SIWZ

2. If the tender is considered to be the most advantageous, the contractor submits: - a) a copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion based on art. 24 sec. 5 points 1 of the Act;

b) informafrom the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids; - c) a current certificate from the competent head of the tax office confirming that the Contractor is not in arrears with taxes, issued not earlier than 3 months before the deadline for submission of bids, or another documentt confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of such receivables, together with any interest or fines, in particular obtained legally required exemption, deferral or installment payment of overdue payments or suspension in full of the execution of the decision of the competent authority; certificate of the relevant local organizational unitof the Social Security Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of those due(e) the Contractor's statement that he has not been the subject of a final judgment of the court or a final administrative decision regarding his resignation, including any interest or fines, in particular obtaining the legally required exemption, postponement or spreading overdue payments; with payment of taxes, fees or contributions onsocial security or health insurance, or in the event of issuing such a verdict or decision documents confirming the payment of these claims together with any interest or fines or conclusion of a binding agreement regarding repayment of such receivables; f) declaration of the Contractor about the lack of a ruling against him as a preventive measure about public ordersne;

g) declaration of the Contractor about non-payment of taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716).

3. The above documents are submitted by each contractor jointly applying for the contract, the entity providing resources and subcontracts. The contractor may apply for the award of the contract, who:

Zadanie 1 (Gdańsk): The Contractor has financial resources or creditworthiness for at least PLN 200, 000. Task 2 (Sopot): The Contractor has funds or creditworthiness for the amount of at least PLN 100, 000. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents indicated in point IV, no grounds for exclusion, paragraph paragraph 2:

- lit. b) - consistsinformation from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence is the person concerned by the information or document, to the extent specified in art. 24 sec. 1 point 13, 14 and 21,

- lit. a), c) and d) submit the document or docin the country in which the Contractor has his registered office or place of residence, confirming that: - he is not in arrears with taxes, fees, social security or health insurance contributions or that he has entered into an agreement with the competent authority regarding repayment of such receivables, including any interest or fines, in particular obtained the legally required exemption, deferral of lor spreading overdue payments into installments or suspending the execution of the decision of the competent authority in full, - no liquidation has been opened or bankruptcy has been announced. Documents referred to in paragraph 2, should be issued not earlier than 6 months before the deadline for the submission of offers (documents for no criminal record and liquidation, bankruptcy) or not earlier than 3 months before the expiry of thisabout the deadline (document regarding the absence of tax arrears, fees, etc.). More information is available in SIWZ.

4. 1. The Contractor may, to confirm compliance with the conditions of participation in the proceedings, where appropriate and with regard to a specific order, rely on the technical or professional capacity or financial or economic situation of other entities, regardless of the nature of therelevant legal relationship.

2. The Contractor, which relies on the abilities or situation of other entities, will prove to the Ordering Party that, when performing the order, he will have the necessary resources of these entities, in particular by presenting the obligation of these entities to give him the necessary resources to fulfill the order; commitment or other document afterdeclaring the availability of resources, should indicate the information indicated in point 8. The contractor together with the offer submit a statement of the entity providing resources, and if the offer is considered the most favorable documents confirming the lack of grounds for exclusion indicated in Chapter VI of this IDW

3. In the case of acting by proxies - the contractor attaches the power of attorney. happinessdetailed information in the ToR.

4. Each Contractor submitting an offer is obliged to secure it with a bid bond in the amount of: Part I PLN 15, 000 (in words: fifteen thousand zlotys), Part II: PLN 5, 000 (in words: five thousand zlotys).
