Sanitary service consisting in the provision, operation and leaving for use for the duration of the order of portable sanitary systems in military complexes administered by 45 WOG

The subject of the order is a sanitary service consisting of delivery, service and afterleaving portable sanitary systems and shower and washbasin stands in sanitary containers for use during the duration of the order, in military complexes administered by 45 WOG in the following cities: Wędrzyn, Międzyrzecz, Skwierzyna and Gorzow Wlkp. Estimated number of portable cabins, freestanding washbasins and shower and washbasin stands in containers for use inthe course of the service is specified in the description of the subject of the contract (constituting Annex 5 to the ToR) and the price form (constituting Annex 2 to the ToR). The Purchaser reserves the right to exercise the option right referred to in Art. 34 paragraph 5 of the Public Procurement Law

1. The subject of the order is a sanitary service consisting ofsupplying, servicing

and leaving portable sanitary systems and shower and sink stands in sanitary containers for use during the duration of the order, in military complexes administered by 45 WOG in the following cities: Wędrzyn, Międzyrzecz, Skwierzyna and Gorzow Wlkp.

2. The service consists in the delivery, service (service) and leaving for use of:

a) cabins fronttoilet (VIP) equipped with toilet, urinal, sink with water tank, liquid soap or disinfectant dispenser, paper towel dispenser, toilet paper dispenser, mirror, clothes hanger, anti-slip floor, internal lock, free / occupied indicator and women`s / men`s, double ventilation (tank for faeces and toilet cabins), toilet paper, paper towels; nand the duration of the contract together with the service once after each week, including final service;

b) free-standing two-position sinks with a water tank capacity of min. 150 l. Clean water and a tank for drainage-free dirty water, equipped with two water draw-off points, a liquid soap or disinfectant dispenser, a paper towel dispenser, a container / basket for used towels; for a period of timevalidity of the contract with service once a week, including final service;

c) portable toilet cabins (VIP) equipped with a toilet bowl, urinal, sink with water tank, liquid soap or disinfectant dispenser, paper towel dispenser, tray for toilet paper, mirror, clothes hanger, non-slip floor, internal lock, vacant / occupied and women`s / men`s indicator, indouble nylons (faeces tank and toilet cubicles), toilet paper, paper towels; for a period of up to 1 week with final service, (so-called party cabins);

d) free-standing two-position washbasins with a water tank capacity of min. 150 l. Clean water and a drainage tank for dirty water, equipped with two water draw-off points, a liquid soap dispenser or disinfectant liquid, paper towel dispenser, container / basket for used towels; for a period of up to 1 week along with the final service, (so-called party washbasins);

e) performing additional servicing (servicing) of cabins and washbasins left to use for the duration of the order;

f) portable toilet cabins (standard) equipped with a toilet, urinal, toilet paper dispenser, clothes hanger, non-slip floor, zamek internal, free / occupied and female / male indicator, double ventilation (feces tank and toilet cubicles), toilet paper; for a period of 1 day securing troops accommodation areas with daily service;

g) free-standing two-position washbasins with a water tank capacity of min. 150 liters of clean water and a drainage tank for dirty water, equipped with two water intake points, dozosoap or liquid disinfectant dispenser, paper towel dispenser, waste container / basket; for a period of 1 day securing troops accommodation areas with daily service;

h) shower and basin stands in containers, equipped with showers, washbasins, soap dispensers, anti-slip floor mirror above each sink, clothes hangers, electrical installationą equipped with electric shock residual current switches, hermetic internal lighting, instructions for use and use of the sanitary container, trash bin, external platform at each entrance to the container, dimensions min. 0.7 m x 0.7 m, made of waterproof and non-slip material;

i) maximum number of portable cabins, freestanding washbasins and shower-washbasin standscontainers in containers for use during the term of the contract is specified in - 3 and the pricing form. the president of the city competent for the place of rendering services - emptying the tanksoutflow and transport of liquid waste in the municipalities of Sulęcin, Międzyrzecz, Skwierzyna and Gorzow Wlkp.- in accordance with the Act of 13.9.1996 on maintaining cleanliness and order in communes (Journal of Laws of 2018, item 1454).

The Awarding Entity will consider this condition fulfilled if:order amount for a guarantee sum of not less than: PLN 1, 800, 000.00.

The Contractor undertakes to maintain the above scope of insurance throughout the duration of the contract.

In order to confirm that there are no grounds for exclusion from participation in the proceedings, the Contractor whose offer will be at most the assessed person submits:

a) information from the National Criminal Record in the scope specified in art. 24 paragraph1 item 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

b) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments issued not earlier than 3 months before the deadline for submission bids, or other document confirming that the Contractor has entered into an agreement with the competent authoritytax on the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or distribution in installments of overdue payments or withholding in full the execution of the decision of the competent authority;

c) a certificate of the appropriate organizational unit of the Social Insurance Institution or the Cash Register Agricultural Social Insurancehim or another document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids or requests to participate in the procedure, or other document confirming that the Contractor has concluded an agreement with the relevant authority on the repayment of these debts together with any, if anyinterest or fines, in particular, he obtained the exemption provided for by law, deferment or distribution in installments of outstanding payments or withholding in full the execution of the decision of the competent authority;

d) an excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate provisions require entry to the register or records to confirm the absence of substaw exclusion pursuant to art. 24 paragraph 5 point 1 of the Act;

e) the Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or in the case of such a judgment or decision of documents confirming payment of these amounts together withany interest or fines or conclusion of a binding agreement on the repayment of these claims (Annex 7 to the ToR);

f) the Contractor`s statement that he has not been awarded a preventive measure against him by applying for public procurement (Annex 7 to the ToR);

the Employer requests from the Contractor, which relies on the abilities or situation of other entities / subcontractor, whomintends to entrust part of the contract, which is not an entity on whose abilities or situation the Contractor relies - on the principles set out in art. 22 a of the PPL, assumptions of documents confirming the lack of grounds for exclusion, i.e. point a-f.
