Subject of the contract:

Subject of the contract:

The subject of the contract are comprehensive cleaning services at the Hospitalat the Wojewodzki John Paul II in Bełchat-w.

2. Code and name according to the Common Procurement Vocabulary (CPV):

90.90.00.00-6 Cleaning and disinfection services

3. Description, size or scope of the subject of the contract:

1) comprehensive cleanliness services will be provided by the contractor to the contracting authority on the terms set out in the model service contract (Annex 3a to the ToR);

2) contractorlease from the contracting authority necessary for the implementation of the subject of the contract utility rooms on the conditions set out in the model agreement for the lease of premises (Annex 4 to the ToR);

3) the contractor will be required to perform the services that are the subject of the contract in accordance with the detailed requirements of the contracting authority set out in:

a. specifications for maintaining cleanliness in medical cellsadministrative and administrative entities (Annex 5a and 5b to the ToR),

b. the terms of the contract (Annex 6 to the ToR),

c. description of the subject of the contract in the field of servicing the rooms of ward ovens, underwear, auxiliary activities at the patient and transport at the hospital (Annex 7 to the ToR);

4) services constituting the subject of the contract will be provided by the contractor withusing means, equipment and devices provided by the contractor. The contractor`s obligation is, among others, to equip ward ovens with the necessary equipment and devices so that each cooker has complete equipment, whose composition is specified in the service contract. Part of the equipment and devices owned by the contracting authority, which is included in the equipment of cookers sectionand in other locations within the Hospital and is suitable for use, the contracting authority will provide the contractor with the opportunity to use them in the implementation of the contract.

5) the total area intended for cleaning is 37 489.22 m;

6) to perform services as part of the contract the contractor will only use disinfecting and cleaning agents that meet the specificationsrequirements by the contracting authority (Annex 8 to the ToR);

7) the contracting authority indicates the structure of the pavilions in which the cleaning services will be provided (Annex 9 to the ToR) and the site plan (Annex 10 to the ToR).

Not applicable! The Employer did not specify the conditions in this respect.

The conditions in terms of economic or financial standing will be considered met if theca shows that:

a. has financial resources or creditworthiness in an amount equivalent to at least PLN 1, 000, 000.00; based on the document referred to in Chapter VI, point I, 2.1 of the ToR;

b. is insured against civil liability in the scope of conducted activities related to the subject of the contract for a sum insured equivalent to the amount of at least 1 0PLN 00, 000.00, based on the document referred to in Chapter VI, point I, 2.2 of the ToR. Journal of the European Union. If, on the day of publication of the contract notice, the NBP does not publish the average exchange rate of a given currency, as a basisconversion, the average exchange rate published on the first day after the date of publication of the contract notice in which it will be published is assumed.

I. Single European Order Document (EAT)

The contractor shall attach to the offer a valid statement as at the date of submission of the offer in the scope indicated by the contracting authority in the contract notice or in the specification of essential terms of the contract. inthe formations contained in the statement constitute preliminary confirmation that the contractor:

1) is not subject to exclusion

2) meets the conditions for participation in the procedure.

This statement the contractor submits in the form of a single European order document (abbreviated "EAT") in electronic form with a qualified signature electronic; the sample statement in the form of EAT in .doc format is attachedNo. 11 to the ToR.

II. Statements or documents confirming no grounds for exclusion

1. The contracting authority will exclude the contractor from the contract award procedure based on art. 24 paragraph 1 point 12 23 of the Public Procurement Law, and in the case of Art. 24 paragraph 5 to the extent that the contracting authority envisaged the possibility of excluding the contractor in accordance with chapter VA of the ToR. The awarding entity shall exclude from the procurement procedurea contractor pursuant to art. 24 paragraph 5 points 1 and 8 of the PPL Act

2. In order to confirm that there are no grounds for excluding the contractor from participation in the procedure, the contracting authority requires the following documents:

1) a certificate from the competent head of the tax office in accordance with Chapter VI of the ToR;

2) a certificate from the appropriate organizational unit of the Social Insurance Institution or Kasa Rolniczego UbezpiecSocial Security or other document in accordance with Chapter VI of the ToR;

3) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids or requests to participate in the procedure;

4) an excerpt from the relevant register or from the central register and information on its activitiesfor business, if separate provisions require an entry in the register or a register to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act;

5) the contractor`s declaration of non-payment of local taxes and fees, referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws of 2016, item 716); the sample printout of the statement is attached as Annex14 to the ToR;

6) the contractor`s statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or in the case of such judgment or decision of documents confirming the payment of these amounts together with interest or fines, if anyof the binding agreement on the repayment of these receivables, the specimen print of the statement is attached as Annex 15 to the ToR;

5) the contractor`s statement that he has not been issued a preventive measure against the prohibition of applying for public procurement, the specimen print of the statement is Annex 15 to the ToR;

6) declarations of the contractor about belonging or not belonging to the same group of kapitałowej; in the case of belonging to the same capital group, the contractor may submit, together with the statement, documents or information confirming that links with another contractor do not lead to distortion of competition in the proceedings, the template for printing the statement is Annex 16 to the ToR.

bid bond. The contracting authority requires contractors to lodge a security deposit.

2. The bid bond depositors are obliged tocarry before the bid submission deadline.

3. The contracting authority sets the amount of the bid bond in the total amount of PLN 200, 000.00 (say: two hundred thousand zlotys 00/100).
