The subject of the order is the service involving the performance of maintenance works on natural watercourses and water facilities, reservoirs and flood embankments afterof measures to free watercourses and maintenance of water facilities works in accordance with the item list for individual sections. Any damage resulting from the service and its removal lies with the Contractor at his own expense and risk, including any third party claims in connection with the execution of the order.

1.1. Conservation of water from Czarnocin, gm. Złota

W km 3 470-5 270:

1. making vegetation from slopes, bottoms, shoulders,

2. removal of shrubs from slopes up to 10 years old, point-by-point with the removal of residues using a chipper 0, 16 ha;

3. removal of trees with circuits not exceeding those allowed in art. 83f par. 1 point 3 of the Actabout nature protection 15 pcs.

4. extraction from the subversion channel and scrap of trees on Wed. 10-25 cm 18 items,

5. removing obstacles from applied branches, impurities;

6. undercutting of willow branches 15 pcs.

7. mechanical removal of overhangs from slopes, layer of gr. 20 cm with straining of the excavated material, width of the bottom up to 1.0 m in km 3 780 - 4 070;

8. planting management with sowing

1.2. Conservation of the Watercourse from Skorocic, gm. Wiślica

In km 2 100-5 000:

1. making vegetation from slopes, bottoms and shoulders;

2. removal of shrubs from slopes up to 10 years old, point-by-point with the removal of residues using the wood chipper -0.09 ha;

3. removal of trees with circuits not exceeding those allowed in art. 83f par. 1 point 3 of the Act on Nature Conservation 6 items

4. manual removal of silt from the bottom with a layer of gr. 20 cm with manual layoutthe excavation of spoil, the width of the bottom 1.5 m at km 4 620-5 000;

5. mechanical removal of silt from the bottom layer of gr. 30 cm with planing of the excavated material, width of the bottom to 2.0 m in km 4 160-4 620;

1.3. Conservation of water from Broniny, gm. Busko-Zdroj

In km 0 000-2 500:

1. plant makingfrom slopes and shoulders;

2. removal of shrubs from slopes, troughs up to 10 years old, point-by-point with removal of the remains using a wood chipper - 0.15 ha;

3. mechanical removal of silt and overhangs with slopes with a layer of gr. 20 cm with manual straining of the output, bottom width up to 2.0 m in km 0 240-1 530; 1 690-2 400;

4. cleaning passages from silt;

5. removing recesses from branches and impurities;

6. fromplanting management;

7. sowing scarps and plant trees with a mixture of grasses;

At km 5 100:

1. excision of two willows that threaten buildings and power lines with the use of a 2-unit increase (they do not represent commercial value)

Purchaser in this proceeding, pursuant to art. 24aa paragraph 1 of the Act will first examine and evaluate offers, and then examine whether the Contractor whose offer will bethe constant assessed as the most advantageous is not subject to exclusion from the proceedings and meets the conditions for participation in the proceedings. Contractors are subject to exclusion pursuant to art. 24 sec. 1, 5 and 7 of the Act. The Ordering Party may exclude the Contractor at any stage of the procedure for granting the order. In order to verify the fulfillment of the conditions of participation in the present proceedings and to demonstrate the lack ofat the basis to exclude the Contractor from the proceedings for the order in the circumstances referred to in art. 24 sec. 1, 5 and 7 of the Act, the Contractor based on art. 25a para. 2 of the Act shall submit to the indicated e-mail address of the Ordering Party valid as of the day of submission of offers a statement in electronic form in the form of a Single European Order Document (EED) signed by the Contractor of the qualselectronic signature - drawn up on the conditions set out in point 2 of this chapter. The document should be prepared in accordance with the specimen standard form specified in the Implementing Regulation of the European Commission. The information contained in the single European order document will constitute a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions of thighsin the proceedings. At the request of the Employer in the mode of art. 26 par. 1 of the Act, the Contractor whose tender has been rated the highest, and the Contractor is not subject to exclusion from the proceedings and fulfills the conditions for participation in the proceedings submits the remaining documents that should confirm "in the present tense" that the actual state is what the document certifies, i.e. day of assembly of the axisor the documents. The orderer reserves the right to carry out the actions resulting from art. 26 par. 2f of the Act. In order to confirm the lack of grounds for exclusion from the procedure for awarding the Contractor's order in the circumstances referred to in art. 24 sec. 1 point 23 of the Act and in accordance with art. 24 sec. 11 of the Contractor's Act within 3 days from posting on the website (www.wodypolskie.bip.gov.pl) - information referred to in art. 86 par. 5 of the Act, provides the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 acts according to the template constituting Annex No. 7 to the ToR. The original document required. In the case of Contractors jointly applying for the award of a document-this one submits each of the Contractors separately. The Contractor may submit together with the declaration documents or information confirming that connections with another Contractor do not lead to distortion of competition in the proceedings. Documents and declarations required from the Contractor at the stage of submitting bids:

1.1. Completed submission form in a written form according to the template specified in Annex No. 1 to the ToR. Required formthe original document; 1.2. Single European Order Document (EAT) according to Annex No. 5 to the ToR, in electronic version filled out and signed by a qualified electronic signature, and also in the scope referred to in the following items. 1.3.1-1.3.3 if there are such "premises" and prepared on the terms set out in point 2 of this chapter.e own Contractor that he meets the conditions for participation in the proceedings regarding technical or professional capacity and that he is not subject to exclusion. NOTE: If the Contractor submits an offer for more than one part of the order - he will prepare only one document on EAT. 1.3. Declaration on the fulfillment by the Contractor of the information obligations provided for in Article 13 or art. 14 GDPR in writingaccording to the formula specified in Annex No. 2 to the ToR. Required form of the original document.

CONTINUATION OF SECTIONS III.1.1)

The Contractor is obliged to complete the EQUIPMENT in such a way that the Employer is able to verify whether the Contractor meets the conditions for participation in the proceedings and is not subject to exclusion to the extent described in these ToR. 1.3.1. In the case of Contractors jointly applying forEach of these Contractors jointly applying for the order shall prepare a separate, uniform document (EED) in an electronic version - completed and signed by a qualified electronic signature on the principles set out in point. 2 of this chapter. These documents are to confirm compliance with the conditions for participation in the proceedings, no grounds for exclusion to the extent that each of the Contractors-demonstrates compliance with the conditions of participation in the proceedings and no grounds for exclusion. If an offer is submitted for more than 1 lot of orders, only one EAT.1.3.2 is prepared. In the case when the Contractor refers to the resources of other entities in order to demonstrate the lack of grounds for exclusion and compliance with them to the extent to which they refer to their resources of thigh conditionsthe proceedings are also submitted - a single document (EAT) in an electronic version drawn up by these entities completed and signed by a qualified electronic signature by the resource provider on the principles set out in point. 2 of this chapter. In the case when the Contractor's reference to the resources of other entities concerns one entity that shares resources, when submitting an offer-for more than one part of the order one EAT is prepared. If, when submitting an offer for more than one part of the contract, and the Contractor refers to resources of at least two entities providing resources, one EIN shall be prepared separately for each entity that shares resources. 1.3.3. In the event that the Contractor intends to entrust the execution of a part of the orders to subcontractors for the purpose ofsermons of lack of grounds for exclusion from participation in the proceedings submit uniform documents (EAT) concerning subcontractors in electronic form filled out and signed by a qualified electronic signature by a subcontractor (s) on the terms set out in point 2 of this chapter. If the Contractor submits an offer for more than 1 lot of orders, only one is preparedn EAT separately for each subcontractor. Form required - electronic version with a qualified electronic signature - prepared on the principles set out in point. 2 of this chapter Instructions for completing the Single European Order Document (EIN) constitute Annex No. 6 to the Terms of Reference. In addition, the Contractor shall attach to the offer (if necessary): 1.4. The commitment referred to in Chapter XI point 2 of the ToR - in the case when the Contractor relies on the resources of other entities in the mode of art. 22a of the Act, the required form dthe original item.1.5. Power of attorney to represent all Contractors jointly applying for the order, possibly a cooperation agreement with which will result from the appropriate authorization to represent in the procedure and signing the contract. The form of the document is the original or notarized copy. 2. Rules for the preparation of the Single European Order Document (EAT) in the electronic versiontronic. 2.1. At the disposal of art. 15 para. 2 of the Act of 22 June 2016. on the amendment of the Public Procurement Law Act and some other acts (Journal of Laws of 2017 item 1579) and art. 10a of the Contractor's Act, participating in the proceeding, submit the Single European (e) Order Document (s) (EAT) referred to in the above sub-item. 1.2 and in the scope of entities referred to in points1.3. 1.3.3, each in the electronic version with a qualified signature
