Due to the limited number of characters in individual fields in the form, the conditions for participation in the procedure and no grounds for exclusion are listed here. Numbering in accordance with the order documents (SIWZ).

3. The contracting authority does not provide for the exclusion of contractors based on the provisions of art. 24 paragraph 5 of the Public Procurement Law

4. Contractors may jointly apply for the contract. In this case, the Contractors shall appoint a representative to represent them in the procurement procedure or to represent them in the procedure and to conclude the public procurement contract. Power of attorney in the formin writing (the original or a copy certified by a notary public to be true copies of the original) must be attached to the offer.

5. The contracting authority will exclude from the proceedings contractors who do not show that they do not have the conditions specified in art. 24 paragraph 1 point 13-23 of the Act;

2.List of declarations or documents to be provided by the contractors with the offer in order to confirm compliance with the conditions for participation in the procedure, as specified in Chapter VI, and to show that there are no grounds for exclusion:

1) current statement as at the date of submission of the offers in the scope indicated in the ToR. The information contained in the statement will constitute preliminary confirmation that the contractor is not subject to exclusionfrom the proceedings and meets the conditions for participation in the proceedings. This declaration is made by the contractor in the form of a single European order document (EAT) prepared according to a standard form specified in the Executive Regulation of the European Commission - Annex 2 to the ToR. Instructions for completing the EAT are available on the website of the Public Procurement Office

2) In the case of joint application for the Contract by the Contractors, the declaration (EAT) referred to in item 1) is made by each of the contractors jointly applying for the contract. This statement is intended to confirm that the conditions for participation in the proceedings have been metonce there are no grounds for exclusion to the extent that each of the Contractors demonstrates compliance with the conditions for participation in the procedure and no grounds for exclusion.

3) The Employer does not request that the Contractor who intends to entrust the performance of part of the contract to subcontractors in order to demonstrate the absence of grounds for exclusion from the participation in the proceedings he made the statement referred to in items 1.

3. The Contractor, within 3 days from the date of posting on the purchasing platform the information referred to in Article 86 para. 5 of the Public Procurement Law, (Information from the opening of bids) provides the contracting authority with a declaration of belonging or not belonging to the same group.enter in accordance with art. 24 paragraph 11 of the Act. Along with submitting the statement, the Contractor may provide evidence that the links with another contractor do not lead to a distortion of competition in the procurement procedure - Annex 3 to the ToR must be submitted via the Purchasing Platform and signed by a qualified electronic signature.

In accordance with art. 24aa section 1 of the Public Procurement Lawthe applicant will first evaluate the offers and then examine whether the Contractor whose offer has been assessed as the most advantageous is not excluded and fulfills the conditions for participation in the procedure.
