The subject of the orders are laboratory supplies, software (dot part No. 1), as well as conducting training on the use and operation of the delivered equipment (regarding the part No. 1

Prthe order is based on the supply of laboratory equipment together with the software, as well as conducting training on the use and service of the delivered equipment, i.e. an Ultra-high performance liquid chromatograph with triplex quadrupole mass spectrometer. A detailed description of the subject of the order can be found in the tender documentation.

In the public procurement procedure, I can participateą take contractors who are not subject to exclusion from the public procurement procedure for reasons specified in art. 24 sec. 1 Pzp. The Contractor shall attach to the offer, including the provisions of point 6.3 of the Terms of Reference: current declaration on the day of submitting offers in the form of a Single European Order Document Form (EED). On request, based on art. 26 par. 1 PPA performer consists of:

- information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13), 14) and 21) PPL, issued not earlier than 6 months before the deadline for submission of bids,

- Contractor's statement that the court has not been the subject of a final court decision or final administrative decision on default of taxes, fees or fees social or health insurance albecause in the case of issuing such a verdict or decision - documents confirming payment of these receivables together with any interest or fines or conclusion of a binding agreement regarding repayment of such receivables, - Contractor's statement about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders.

If the Contractor is established or haveresidence outside the territory of the Republic of Poland, instead of the documents referred to in point 6.4.1.1. The ToR, shall submit information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence has the person to whom the information relatesmation or document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 PzP. - Documents referred to in item 6.6. The ToR, should be issued not earlier than 6 months before the deadline for submission of tenders.

If in the country in which the Contractor has a registered office or place of residence has the person concerned, the documents referred to in item 6.6 are not issued .The SIWZ is replaced by a document containing the Contractor's declaration, indicating the person or persons authorized to represent it, or a statement of the person the document was to be submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authority or the place of residence of the contractor or methe residence of that person. The document referred to in the preceding sentence should be issued not earlier than 6 months before the deadline for submission of tenders. - Contractor established in the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, which is covered by the document indicated in point 6.4 .1.1 SIWZ, submits the document, o-referred to in item 6.6 of the Terms of Reference to the extent specified in art. 24 sec. 1 items 13, 14 and 21 PPL. If there is no such document in the country in which the person concerned has a place of residence, it is replaced by a document containing the person's statement made before a notary or before a judicial, administrative or self-regulatory bodydue to the place of residence of that person. The document referred to in the preceding sentence should be issued not earlier than 6 months before the deadline for submitting bids.

Not applicable - The Contractor will attach to the offer, taking into account the provisions of point 6.3 SIWZ: valid as of the date of submitting bids, a Form in the form of a Single European Order Document (hereinafter referred to as "EAT"). ANDthe information contained in the EO is a preliminary confirmation that the contractor is not subject to exclusion (point 5.2 of the ToR) and meets the conditions for participation in the proceedings (point 5.1 of the ToR).

The Contractor joining the present proceedings is obliged to pay the bid bond for the time of binding the offer. secured bid bond in the amount of: - Task No. 1: PLN 25, 000.00 (in words: twenty five thousand zlotys 00/100), - Task No. 2: PLN 1, 000.00 (in words: one thousand zlotys 00/100), - Task number 3: PLN 1, 500.00 (in words: one thousand five hundred zlotys 00/100),

In the case of submitting a partial offer, the contractor is obliged to pay a deposit in the amount specified for the given task. In the case of submitting an offer for several tasks, the amount of the deposit is the sum of wadiow determined for individual parts of the order (for poszczmain tasks). If the bid security amount is lower than the amount resulting from individual parts of the order, the orderer will consider that the bid security has not been paid in the required amount.

The required bid bond must be submitted before the deadline for submission of offers.6.10.

Contractor, within 3 days from the date of placing on the website of information - From the opening of offers, referred to in art. 86paragraph. 5 of the PPL, shall provide the Employer with a statement on the affiliation with or lack of belonging to the same capital group as referred to in art. 24 sec. 1 point 23) Pzp.
