1. The subject of the contract is a service for the daily and all-day preparation and delivery of meals for patients of SP ZOZ in Sulech-w.

1. The subject of the order is a service for the daily and all-day preparation and delivery of meals for patients of SP ZOZ in Sulech-w.

2. The Contractor will be obliged to provide mass catering services in health care observing the principles of good hygiene practice, principles of good production practice and apply the implemented HACCP system principles. The contractor will be required toalso during the implementation of the order, ensure the quality of food in accordance with the recommendations regarding standards and nutrients and food products assigned to healthcare facilities, including diets, meal preparation technologies, issued by the Food and Nutrition Institute in Warsaw, especially in terms of nutritional value, variety, volume and weightfog. Meal production and distribution should guarantee sanitary and epidemiological safety. Meals should be produced from fresh, top-quality products that have documents authorizing them to be eaten, maintaining dietary and sanitary regimes applicable in hospitals in accordance with applicable regulations.

3. Scope of providing services:

1) Preparecooking and cooking meals according to the norms and principles of nutrition assigned to healthcare facilities, in amounts resulting from the state of hospitalization, taking into account dietary recommendations, and paid meals for the employees concerned, in accordance with the principles of the HACCP system:

- calorific value not less than 2, 300 calories,

- temperature of meals delivered:

a) sausages from 0 to 4C;

b) dairy products from 0 to 8C;

c) soup not less than 70C;

d) potatoes, rice, cereal, etc. not less than 70C;

e) meat, stews not less than 65C;

- level of nutrients:

a) in accordance with the sample menu attached No. 3 to the Terms of Reference;

• at least 3 times a week, a whole portion of meat should be served (including on Sunday, obligatory), no stew and meatballs;

• for special diets, 2 types of salads, may be equalthat vegetables after heat treatment;

• for the diabetic diet and afternoon tea should be varied with different products and not roll over the next 5 days;

• in breakfast and dinner menus, apart from basic ingredients, there must be additionally fresh vegetables, fruits or their preparations;

• rye, wheat or wholemeal bread (according to ordered diets);

• for spreading bread onto deliver butter

• meals should be supplemented with additions such as salt, sugar, mustard, ketchup, etc. (delivered in daily portions, according to the orders of the meals). Supplementing meals with additions will take place at the portioning stage

• prepared meals must comply with all recipe, taste, aesthetic and health requirements with particular regard tokeeping standards of daily nutrition rations for health care facilities.

2) Delivery of meals to the Ordering Party to the following addresses: Sulechow, ul. Zwycięstwa 1 and al. Niepodległości 19.

Delivery of containers with meals to wardrobes should take place in a manner that guarantees the preservation of proper temperature, cleanliness and quality of meals, i.e. using transport carts, containers, bemaro and other appropriate devices of this type.

a) After unloading the meals, empty thermoses, etc. are immediately forwarded to the driver.

b) The Ordering Party is responsible for proper maintenance of ward stoves. Separation of meals belongs to the Employer.

C) The Employer provides tableware in addition to the equipment listed in point I.3. item 12) and 21) which remains the property of the Contractor.

d) The Employer does not haverooms that can be made available to the Contractor for the purposes of the delivery of meals. transport because the Employer does not have the option of storing meals.
