Part I The subject of the order is a systematic provision of work to the Employer, up to a maximum of 38, 000 man-hours, consisting of:

The object of the order is a systematic provision of work to the Ordering Party, with a maximum of 38, 000 man-hours, consisting of:

1. Movement (lifting, lifting, stacking, transporting, moving, moving, shifting), objects, loads or materials, in particular electronic equipment (eg: drdevices, computers, monitors), household appliances (eg: TV sets, mobile air-conditioners, coffee machines), furniture (eg: desks, chairs, cabinets, tables) and other items (eg: interior design elements).

2. Loading and unloading of vehicles at the disposal of the Employer.

The detailed description of the subject of the order is attached as Annex 1a to the ToR.

The Contracting Authority provides for the possibility of granting-orders referred to in art. 67 sec. 1 point 6 of the Act, consisting in repeated similar services up to 50% of the value of the basic order.

Performing activities in the scope of order fulfillment indicated in point. I OPZ - Part I economic work (Annex 1a to the Terms of Reference), consists in performing work in the manner specified in art. 22 - 1 of the Act of 26.6.1974. Labor Codeand requires employment by the Contractor or a subcontractor on the basis of a contract of employment of full-time employees.

Deadline for ordering: 24 (say: twenty-four) months from the date of signing the contract (but not earlier than from 15/10/2018) r.) or until the financial resources provided for the maximum gross remuneration (- 3 ust. 1 of the contract), subject to - 4 of the contract, if these funds will be used on an earlier date. The contractors who are: - 1.1 are not subject to exclusion from the proceedings under Art. 24 sec. 1 of the Act,

1.2 meet the conditions for participation in proceedings in the field of technical or professional capacity.

2. In order to confirm compliance with the conditions of participation in the progressas well as demonstrating the lack of grounds for exclusion, the contractors are obliged to submit together with the offer:

2.1. Current on the day of submitting bids a statement in the scope indicated by the Contracting Authority in the SIWZ, in the form of a single document (EAT). EAT should be submitted separately for each part of the order.

1) Information contained in EED will constitute preliminary confirmation that the Contractor is not subject to exclusionfrom the proceedings and meets the conditions for participation in the proceedings.

2) The Contractor submits the GAS according to the standard form set out in the Commission Implementing Regulation (EU) 2016/7 of 5.1.2016, in electronic form with a qualified electronic signature to the e-mail address mail: bzp@msz.gov.pl

3). Detailed information on the EED document can be found in the order documents-(SIWZ).

2.2 If the Contractor is reliant on the ability of other entities to oblige these entities to place the Contractor at the disposal of the necessary resources to perform the order or other evidence confirming that when executing the order, the Contractor will have the necessary resources of these entities. The obligation of another entity is submitted in the original.

2.3 In the case of Contractors-jointly applying for granting the order, the power of attorney to represent the Contractors in the procedure or to represent in the procedure and conclusion of the contract regarding the order, submitted in writing (the original or a copy certified for compliance by a notary).

3. Contractor within 3 days from the date of posting on the Employer's website information referred to inArt. 86 par. 5 of the Act (information from the opening of offers), is obliged to provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Act, with Economic Operators who submitted separate offers in the subject proceedings.

1) Together with the submission of a statement, the Contractor may submit documents or information on the moniesthat the links with another Contractor do not lead to distortion of competition in the proceedings.

2) In the case of Contractors jointly applying for the order, the declaration is submitted by each of the Contractors.

3) The declaration is made in the original.

4. Detailed information is contained in the order documents (SIWZ).

1. The ordering party, before granting the order, will call the Contractor whose bidrta was the highest rated in a given part of the order, to submit in the prescribed, not shorter than 10 days, the date, as of the day of submitting the following statements or documents:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids;

2) Contractor's statementon the lack of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or - in the event of such a verdict or decision - documents confirming the payment of such claims together with any interest or fines or conclusion binding agreement on repaymentt of these receivables;

3) declaration of the Contractor about the lack of a decision against him as a preventive measure of the prohibition to apply for public orders.

2. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the document referred to in point 1.1), the Contractor shall submit information from the relevant register or, in the absence of such a register, anothera valid document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence is the person concerned by information or a document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Act

3. If in the country in which the Contractor has his registered office or place of residence or place of residence has the person concerned dthe document, referred to in point 2, does not appear to be replaced by a document containing the Contractor's statement, indicating the person or persons authorized to represent it, or a statement of the person to whom the document was to be submitted, before a notary or before a judicial, administrative or the authority of the professional or economic self-government with jurisdiction over the seat or-place of residence of the Contractor or place of residence of this person.

4. The Contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, which concerns the document indicated in point 1.1) above, submits the document referred to in point 2 to the extent specified in art. 24 sec. 1 point 14 and 21 of the Act.If in the country, in-whose place of residence is the person to whom the document was to be related, such documents are not issued, it is replaced by a document containing a statement of that person made before a notary or before a judicial, administrative or professional self-government or economic authority competent for the place of residence of that person. 5. The documents referred to in points 2-4 should besubmitted no earlier than 6 months before the deadline for submission of tenders.

6. Documents prepared in a foreign language are submitted together with a translation into Polish.

7. Other requirements regarding declarations and documents are included in the order documents (SIWZ).

8. The contracting authority may, pursuant to art. 24aa of the Act, first evaluate the offers, and then examine whether the contractor whose offer will berated as the most advantageous, it is not subject to exclusion from the proceedings and meets the conditions for participation in the proceedings.

9. The Contracting Authority requires a bid bond in the amount of: - Part I PLN 35, 000.00, - Part II PLN 3, 000.00.

10. Within 7 days of receipt of the information about the selection, the selected Contractor shall provide the Employer with a list of names (name and surname), PESEL number) addressed to the implementation of the contractincluding confirmed copies of documents confirming employment on the basis of an employment contract (Part I), confirmed for compliance with the original, in order to verify them before being allowed to carry out work on the premises of the Employer, referred to in - 8 IPU on Part I of the order and in - 6 IPUs for Part II orders.
