Supply of negotiation of framework agreement regarding service - and logistics services for TeleCare Nord

TeleCare Nord provides framework agreement for delivery of Operations, Logistics and Services in reTeleCare Nord's operation of cross-sector telemedicine on behalf of the Region of North Jutland and the 11 northern Jutland municipalities.

TeleCare Nord has been established as a cross-sectoral cooperation between Region North Jutland, the 11 North Jutland municipalities and the general practitioners in North Jutland.

TeleCare Nord carries on behalf of co-operation partners, the day-to-day management of settlement and undesupport for cross-sectoral telemedicine issues.

The purpose of the Framework Agreement is to ensure a uniform, stable and common service and logistics solution for all participants in TeleCare Nord, where health professionals and citizens must experience high quality in deliveries wherever Borger lives. The principle is a common coherent patient relationship, with common services and solutions in the NorthJylland.

The Supplier is responsible for the day-to-day operation of the Telemedicine Service in North Jutland as Supplier will be responsible for ensuring that Citizens who are registered with a Telemedicine Company receive a good start-up with a timely delivery of TeleKit and setup and possible upload by TeleKit on Borgers own residence.

The suppliers will thus handle orders from approx. 100 SUndesirable in all 11 municipalities and from Nordjylland.

TeleCare Nord has 1.8.2018 approx. 1 050 Citizens in North Jutland who receive telemedicine home monitoring via a TeleKit.

Citizens send their telemedicine data to a health professional in the municipality or at the hospital 1 to 2 times a week, which evaluates data and assesses whether intervention is needed.

The offer of eThe telemedicine program comprises at the time of delivery respectively



Ca. 900 Citizens with COPD, which is in permanent operation throughout North Jutland, and

- Approx. 150 citizens with heart failure, who run as a project until the summer of 2019, where prior consideration is given to the continuation. Citizens with heart failure are covered by Option 1 Takeover deliveries of Heart TeleKit.

Average age for Citizens with COPD equalgives you approx. 70 years. The majority lives in their own homes. Some live in a care center and receive home monitoring here. The target group for the offer for telemedicine care includes primary citizens with swallows COPD. The function level of this group is often greatly reduced with many limitations in everyday life. For residents with heart failure, there is a greater variation in age, but there are often also talk ofold citizens. As in COPD, heart failure may often have negative consequences for the citizen's functional level and quality of life.

The acquisition project involves the replacement of the existing technically-related TeleKit, which is currently used for Citizens in Telemedicine care, and which is located at the citizen, either at home or elsewhere kind of care home.

The supplies should be sold as part of OvThe takeover project handles the repository of existing TeleKit and replacing this with a new TeleKit, a deliberate Takeover Delivery, where new TeleKit is delivered, installed and Borger is charged for use of this within the milestone laid down in Framework Agreement 1 for the approx. 900 Citizens with COPD throughout northern Jutland, who are in a telemedicine program.

Safely TeleCare Nord in connection with-The completion phase of the completion phase assumes Option 1, the Supplier must accordingly replace TeleKit at approx. 150 Citizens with a heart failure within the milestone reported in Rammeaftalebilag 1.

The takeover project is expected to be carried out with the exchange of existing TeleKit municipal councils by municipality, possibly by postcode, in an agreed "roll" where delivery is determinedAfter the takeover project, TeleCare Nord has replaced all existing technically-owned TeleKit with new updated TeleKit, which can support the anticipated development of telemedicine services under the TeleCare Nord in the framework agreement.

After the completion of the takeover project, LiveThe application shall be made up of the Single European Procurement Document (ESPD), in which the requested information referred to in points III.1.2 and III.1.3 must be disclosed. The ESPD is also a self-declaration that serves as preliminary proof that Applicants, Other Entities or the Association meet the criteria with regard to the outsideaptitude, suitability and selection. This information must be verified later, see Section 3.1 of the Terms of Service. Applicants fill in the ESPD in TenderKONNECT via "Fill ESPD" according to the requested information in points III.1.2 and III.1.3. The document can be saved along the way by clicking "Save ESPD."

Other Units or Associations must apply also fill in an ESPD.

Pa will review the tender documents and TeleCare Nord's expectations for the Framework Agreement and the implementation oftenderer.

Safer bidder wishes to participate in the meeting, the tenderer is asked as soon as possible to convey his interest and names of the participant by sending a questionnaire to TeleCare Nord through the TenderKonnect system.

TeleCare Nord will document the information meeting with a report and address any questions and answers, cf. Section 1.8.2 of the Terms of Service.

Tenderer, Other Entities and Samalgamations must submit to the Ordregiver's request documentation pursuant to section 144 and 152 of the Public Procurement Act for the information provided in the ESPD. Below is the description of how this documentation is to be submitted.

If the Offeror, the Association's Participants or Other Entities are from Denmark, the Proof of Service is provided in the form of a service certificate from the Danish Business Authority.

ServiceThe certificate must not exceed 12 months from the date of submission of the Service Certificate as specified by the Contractor in the letter of invitation.

If the tenderer, one or more of the participants in the Association or Other Units is not from Denmark, this shall, as a rule, provide the types of documentation, as evidenced by e-Certis. e-Certis is an online information system likemust be used to find the right documentation to be used by EU Member States in connection with the tender. Use of the order not in the country in question may be used instead of a declaration of faith and law. Consortia and entities that base their suitability on the Contractor reserves the right to request the Tenderer to confirm that the participation of the same legal entity in multiple offers does not allow reciprocal-affect the content of the different tenderers' offers, including the absence of practices that may threaten transparency and distort competition between tenderers. If this can not be satisfied, the Contractor reserves the right to reject such tenderers
