The subject matter of the contract is a special medical material for the Valvular Heart Clinic. Aortal or mitral valves are intended to replace valves in patients with impaired or otherwise unjustly functioning aortal or mitral valves.flap. They can also be used as a replacement for a previously implanted prosthetic heart valve. - Pharmacy, Ondavska 8, 040 11 Kosice, Slovakia.abrasion into aortal and mitral position with a minimum angulation of 85, with a leaf guard system,

- biological stent valve prostheses with anti-calcification treatment, with ultra-short wash times of 2 x 10 seconds, porcine to aortal and mitral positions and pericardial hovaedias with porcine endocardium covering the titanium stent to the aortal position,

- aortal conduit with designValsalva with rotatable valve prosthesis with an opening angle of at least 85, with graft length min. 10 cm

1. 1 ZVO. Their fulfillment according to - 32 par. 2 a public procurement contract by submitting scanned original documents or a certified copy thereof. 2. The bidder shall demonstrate compliance with the conditions of participation under point 1. as follows:

a) pism. (a) - evidenced by a statement of criminal records not older than 3 months, since the contracting authority is not entitled to obtain them from the public administration information systems pursuant to Act no. 177/2018 Z .z. on some measures to reduce administrative burdens through the use of public administration information systems and to change and supplement some (bureaucracy),

b) pism. b) - attested by pa health insurance company and Social Insurance Agency not older than 3 months,

c) pism. (c) - evidenced by a local tax return not older than 3 months; (d) - attested by a validation of the relevant keg not older than 3 months; (e) - documented proof of entitlement to supply the goods, carry out construction work or provide a service which corresponds to the subject of the contract;f) font. (f) - attested by a declaration of honor, - 3. The applicant may prove that the conditions of participation in the list of economic operators have been fulfilled pursuant to - 152 of the Public Procurement Act.- 4. Pursuant to Section 39 of the Public Procurement Act, the bidder may provisionally replace documents to prove fulfillment of conditions participation by public purchasers through the Single European Doc(hereinafter referred to as JED).

1. Applicants must meet the conditions for participation in the financial and economic position referred to in Article 33 (1). 1 public procurement law. Their fulfillment is proven by submitting scanned original documents or their certified copies as follows:

1.1 - 33 para. 1 pism. a) - Statement by a bank or branch of a foreign bank (hereinafter referred to as the bank) not older than 3 monthse on the date of submission of the bid for the applicant's ability to perform financially. In the statement, the bank confirms that the applicant for the previous 3 years, respectively. for those years for which the Bank's statement may be made, depending on the origin or commencement of the activity, until the date of the Bank's statement:

a) is not / has not been overdrawn;donation,

c) his current account is not / has not been subject to execution

In the event that the applicant has accounts in several banks, the contracting authority requires bank information from each of these banks, in the above-mentioned scope. signed by his statutory body or by another authorized representative of the applicant who is authorized to act on behalf of the applicant in a relationship of interest, indicatinga statement by all banks in which the business has established accounts and at the same time a statement that it does not have bank accounts in banks other than those from which it has submitted a bank statement

2.The bidder or interested may use another person's financial resources to prove financial and economic standing regardless of their true relationship. In such a case, the tenderer or the contracting authority shall be required to:. Tto prove to the purchaser that, when performing the contract or the concession contract, it will actually use the resources of the person whose status it uses to prove its financial and economic standing. The fact, according to the second sentence, proves the applicant or the applicant by a written contract concluded with a person whose sources he intends to prove his financial and economic standing. The written contract must contain a number of axesthat it will provide performance throughout the contractual relationship. A person whose resources are to be used to prove financial and economic standing must prove that the conditions relating to personal status are fulfilled, except for - 32 para. 1 pism. e) and shall not have any exclusions for exclusion pursuant to - 40 par. 6 pism. a) to h) and para. 7.

5 The bidder may, in accordance with Article 39, provisionally replace documents to prove compliance with the conditions of participation specified by the contracting authorities through JED. All the costs and expenses associated with the preparation and submission of the tender shall be declared by the bidder without a financial curse against the contracting authority, irrespective of the outcome of the procurement. 4. The applicant may, in accordance with - 39 of the FTA, replace the documents with a Uniform European Document, in which case he is obliged to fill it in compliance with the FTA. The contracting authority will act in accordance with - 20 of the Public Procurement Act through the Communi cationinterface of the Josephine system. This method of communication concerns any communication and filing between the contracting authority and the bidders / bidders during the whole procurement process. and once the shipment gets toits disposition. For the moment of delivery, Josephine is considered to be the moment of its delivery in the Josephine system in accordance with the functionality of the system. Details are described in the supporting documents.

Submission is only allowedauthenticated bidder. Authentication can be carried out using the following methods:

a) in the Josephine system by registering and logging in using an ID card with an electronic chip and a security personality code (eID). The system is authenticated by a company that registers the statute of the company using the eID. Authentication is done by the system provider Josephine on business days in8.00-16.00 hrs

b) by uploading a qualified electronic signature (for example, signing the eID) of the statutory person of the given company to the User card after registration and logging into the Josephine system. Authentication will be performed by the system provider Josephine on business days at 8 am to 4 pm

c) by placing a power of attorney on the user card after registration, which is signed electronically by the statuteauthorized person or has undergone a guaranteed conversion. Authentication will be done by the system provider Josephine in the working day at 8.00-16.00 hrs

d) by waiting for the authentication code, which will be sent to the address of the firm's address in the hand of the statutory applicant in the form of registered mail. The time limit for this termination is usually 3 business days and it is necessary to count with this deadline on deposit
