Delivery of uniforms and equipment - berets and winter hats

The subject of the order is the supply of uniforms and equipment - berets and winter hats, in the scope of 6 tasks:

Main Site: According to Uniform delivery plan which will be an attachment to the contract concluded.

1. The subject of the order regarding tasksand No. 1 is the supply of uniforms and equipment: berets - in quantity: 27, 210 pieces

2. The contract will be carried out on time in accordance with the offer of the contractor selected to perform the contract (the deadline is one of the criteria for the evaluation of bids). The contractor will offer one of three delivery times. Depending on the offered delivery date, the Contractor will receive a replythe wrong number of points:

- until August 31, 2020, - 20 points,

- until September 30, 2020, - 10 points,

- until November 30, 2020 - 0 points,

Deadline for exercising any options until November 30, 2020

3. In order to confirm that the offered subject of the contract meets the requirements specified by the contracting authority, the contracting authority will require the contractor whose offer is the highest to be submitted within no less than 10 days of the following dockmentow:

1) a document confirming that the contractor`s quality management system meets the requirements contained in: AQAP 2110 edition D version 1

2) current certificate confirming the possession of a factory beret pattern, in accordance with WDTT 418 / MON and the PUiW model (with date of issue later than Date of approval of WDTT 418 / MON by the Commander of the Military Uniform Service Research and Implementation Center)data by the entity authorized to quality control (WOBWSM in Ł-dź) confirming that the offered subject of the order meets the technical specifications specified by the contracting authority;

1. The Employer requires that the Contractor be in an economic or financial situation ensuring the implementation of the contract, which is understood as having financial security in the amount of not less than:

For task # 1: PLN 107, 000.00

For task # 2: PLN 314, 600.00

For task # 3: PLN 15, 800.00

For task # 4: PLN 119, 200.00

For task # 5: 113, 700.00 PLN

For task 6: PLN 33, 200.00

2. If the Contractor intends to apply for the award of several tasks, the amount of financial security held may not be less than the sum of the amount required for individual tasks.

3. To initially confirm that the condition of participation in the proceedings regarding economic or financial situation is met, the contractor fills in section α (alpha): a general statement regarding all qualification criteria in Part IV EAT (does not need to complete any of the other sections in Part IV).

4. Contractor, the highest bidder will be asked to submit a denominationwithin a deadline of not less than 10 days, documents confirming compliance with the abovementioned condition, i.e.: information from the bank or cooperative savings and credit union, confirming the amount of financial resources or creditworthiness of the Contractor in the period not earlier than 1 month before the deadline for submitting offers. If for a valid reason the contractor cannot submit youdocuments, the contracting authority allows the contractor to submit other documents that sufficiently confirm that the described condition of participation in the procedure has been met.

5. The contractor may, in order to confirm compliance with the condition of participation in the procedure referred to above, rely on the financial or economic situation of other entities, regardless of the legal nature of the entities connecting himlegal relations. The contractor, which relies on the situation of other entities, must prove to the contracting authority that, when carrying out the contract, he will have the necessary resources of these entities, in particular by presenting the undertaking of these entities to make available to him the necessary resources for the purposes of the contract. The contractor must make a commitment with the offer (original commitmentin the form of an electronic document signed by a qualified electronic signature providing access to the resources). The contracting authority will assess whether the financial resources made available to the contractor by other entities allow the contractor to demonstrate compliance with the condition of participation in the procedure and will examine whether there are grounds for exclusion for these entities, as referred to in art. 24 paragraph 1 and item 5 points1) and 8) of the Public Procurement Law

6.If the contractor refers to the resources of other entities, together with the offer it submits separate EAT forms regarding these entities in order to demonstrate the lack of grounds for exclusion and fulfillment in so far as it refers to their resources, conditions for participation in the procedure (statements by EAT of entities providing resources signed with a qualified signatureby the entity providing the resources).

1. Contractors who are capable of performing the contract properly, meet the conditions for participation in the procedure and are not subject to exclusion pursuant to Article 24 (1) and Article 24 (5) may apply for the award of the contract. items 1) and 8) of the Public Procurement Law

2.EAT declaration in electronic form with a qualified electronic signature must be submittedTogether with the bid, JEDZ shall submit each of the contractors jointly bidding for the order. This statement is intended to confirm that the conditions for participation in the procedure have been met (to the extent that each of the contractors demonstrates compliance with the conditions for participation in the procedure) and no grounds for exclusion. If applicable - EAT must also be submitted for the entity

3.The contracting authority shall call the contractor whose tender has been rated the highest before submitting the contract to submit within not less than 10 days a deadline, valid as at the date of submission of the documents referred to below in points 4 and 5, in section II (in the scope each task) and in section III.1.2) of this notice.

4. To confirm that there are no grounds to exclude the contractor fromthe contracting authority will require the contractor whose bid is highest rated:

1) an excerpt from the relevant register or central register and information on economic activity, if separate provisions require an entry in the register or register, in order to confirm the absence of grounds for exclusion pursuant to Article 24 para. .5 point 1 of the Act;

2) information from the National Criminal Record in the scope specifiedm in Article 24 (1) items 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for submitting bids in the proceedings;

3) a certificate from the competent head of the tax office confirming that the contractor is not in arrears with tax payment, issued not earlier than 3 months before the deadline for submission of bids;

4) a certificate from the appropriate local organizational unit ZaklSocial Insurance or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids;

5) the contractor`s statement that he has not issued a final judgment against him court or final administrative decisionon arrears in the payment of taxes, fees or social security or health insurance contributions, or - in the event of such a judgment or decision-document confirming the payment of such receivables together with any interest or fines, or the conclusion of a binding agreement on the repayment of these receivables;

6) statements contractors about the lack of a decision against him as a measure will preventof the legal prohibition to apply for public contracts.

7) the contractor`s declaration of non-payment of local taxes and fees, referred to in the Act of 12 January 1991, on local taxes and fees (Journal of Laws of 2016, item 716) .

5. The Purchaser will require the contractor, which relies on the abilities or situation of other entities on the principles set out in Article 22a of the Public Procurement Law, to present inwith respect to these entities, the documents listed in point 4,

6. If the contractor intends to entrust the implementation of part of the subcontractor`s contract, the contracting authority requires submitting a EAT declaration of that subcontractor with the offer. Under Article 26 (1), the contracting authority will request the documents referred to in point 4 regarding of this subcontractor.

7. The offer must include a completed offer, price form andpower of attorney (if applicable).

8. To the offer, the contractor must attach the obligation of the entity providing the resources referred to in section III.1.2) point 5 (if applicable).

9. The contractor submits the offer via the purchasing platform. The method of its submission is described in the ToR.
