The subject of the order is the supply of specialized research materials, i.e. enzymes, cell culture fluids, nutrientsek, reagents, proteins, etc. to UMCS in Lublin.

The subject matter of the order is the supply of specialized research materials, ie enzymes, cell culture fluids, nutrient solutions, reagents, proteins, etc. to UMCS in Lublin.

The Contractor will provide the Ordering Party with the delivered on the basis of orders jednostkowych of the order:

- manufacturer's warranty

- warranty of suitability for use, not shorter than 60% of the declared suitability for use provided by the manufacturer (counting from the day of delivery).

The contractor is obliged to accept the packaging from re-use and packaging waste on dangerous goods in order to pass them on to the manufacturer, importer or makingwithin the Community acquisitions. The Employer requires that the Contractor does not charge a deposit for the packaging, due to the fact that the Materials being the subject of the contract will be used for research or for didactic purposes as well as laboratory diagnostics. The subject of the contract covered by the individual order is to provide places each time to indicateordered by the Ordering Party, at its cost and risk within max. 8 business days from the date of ordering, with the exception of the assortment for which a longer delivery period is provided, is extended due to the production cycle or related to the delivery from abroad. In this case, the Ordering Party allows for the extension of the order completion date. The Contractor shall inform the Employer about the anticipated partasie of order fulfillment

The contractor will ensure the quality and suitability of the products sold in accordance with the standards in force in relation to the individual assortment. A detailed description of the object of the contract is included in the Annex to the ToR.

Contractors who are not subject to exclusion pursuant to art. 24 sec. 1 item 12-22, para. 5 point 1, point 4 and point 8 of the Act. List of required documents confirming the fulfillment of the conditions for participation in the proceedings, compliancerequirements specified by the Employer and no grounds for exclusion:

1. Submitted documents with the offer: • A statement in the form of EAT, valid as at the day of submitting bids. The information contained in the statement will constitute a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure.

2. Documents submitted by the Contractor, whose offer will be evaluated as themore advantageous at the request of the Ordering Party: • The ordering party will call the Contractor, whose tender not subject to rejection has been the highest rated, to submit in the prescribed, not shorter than 10 days, the date, valid as at the day of Submission, the following declarations and documents: no grounds for exclusion of the Contractor from participation in the proceedings, the Ordering Partygive the documents referred to in point 6.2 (a) A SIWZ:

1) Information from the NQF in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Act

2) Certificates of the competent US governor confirming that the Contractor is not in arrears with tax payment.

3) Certificate of the relevant ZUS or KRUS organizational unit or other document confirming that the Contractor is not in arrears with paymentsocial security or health insurance contributions, - Issued no earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has made an agreement with the competent authority regarding the repayment of these receivables

7) Contractor's statements on non-payment of taxes and local fees, about referred to in the Act of 12.1.1991 on local taxes and charges.

In order to demonstrate the lack of grounds for exclusion from the proceedings in the circumstances referred to in art. 24 sec. 1 point 23) of the Act, the Ordering Party requires (after opening bids) submission of the Contractor's statement on - Affiliation or non-affiliation to the same capital group. 6. The contractor joining the tender is obliged to make a deposit before the end of the auctionthe deadline for submission of offers in the amount of PLN 15, 000.00
