1. The subject of the order is "Modernization of land and building registers in the area of registration covens: Godow, Gołkowice, Skrbeńsko, Krostoszowice, Łaziska, Podbucze and Skrzysz-w located in the Godow Poviat Wodzisławski registry unit".

The work under the "Project for modernization of land and building records" is to supplement the registration database and createfull range of the registration data in accordance with the requirements of the Regulation and modification of registration data to the requirements specified in the Regulation.

2. Scope of works envisaged for execution:

The subject of the orders covers the implementation of two tasks:

a) Task 1 - all works for registration cadets: Godow, Gołkowice and Skrbeńsko;

b) Task 2 - all works for cadastral enclosures: Krostoszowice, Łaziska, Podbucze and Skrzysz-w.

Each of the tasks will be implemented in two stages.

Stage I:

1) Elimination of discrepancies in the scope of subject data

2) Supplementing the registration database with data on buildings and premises

3) Modernization registration database in the field of built-up land, urban and built-up land, verification of other classification contours

4) Supplementing the land and building registry database with registration data resulting from the provisions of the regulation

5) Loading data into the EGiB test database

6) Completing the technical report

7) Controlling the study

8) Preparing the project a descriptive and cartographic opera for the presentation.

9) Receipt of stage I.

Phase II

1) Presentation of interested persons to the project fordescription and cartography.

2) Completing the annex to the technical report.

3) Transfer of the data developed to the database kept by the Employer.

4) Completion of the work

3. Basic data about the municipality:

1) recordable area: approximately 3, 802.6 ha,

2) number of plots: 14 711,

3) estimated number of buildings, graphic part: 9 834.

4. In detail, the subject of the order is specified in the section-II SIWZ "Description of the subject of the order." 5 The order should be completed within the required time: from the date of conclusion of the contract until 20.5.2020, subject to: Task 1, all works for reference cadets: Godow, Gołkowice and Skrbeńsk. I until 20/05/2019,

- Stage II until 19/08/2019. - Task 2 - all works for registration cadres: Krostoszowice, Łaziska, Podbucze and Skrzysz-w

- Stage I to 20.2.2020,

- Stage II to 20.5.2020.

1. Each contractor is obliged to secure its bid bond in the amount of PLN 5, 000.00 (in words: five thousand PLN 00/100). The bid bond should be lodged on the terms specified in SIWZ - Section I Instructions for Contractors, Chapter X "Bid bond".

2. The contractor whose offer will be selected as the most advantageous will be obliged to bringsecure the due performance of the contract in the amount of 5% of the total gross price specified in the offer (including VAT), in accordance with the provisions of the ToR.

1. Grounds for exclusion: the contracting authority will exclude a contractor from the proceedings in the cases referred to in art. 24 sec. 1 point 12-23 of the Public Procurement Law (grounds for mandatory exclusion).

2. In order to listThe absence of grounds for exclusion from the procedure for granting the order and compliance with the conditions for participation in the procedure specified by the awarding entity in point 3 of Chapter V of the TOR must be submitted at the day of submitting the offers a statement in the form of: Single Form European Order Document (abbreviation: JEDZ) constituting Annex No. 2 to Section III of the SIWZ. Information contained in Formulathe government of EED is a preliminary confirmation that the contractor is not subject to exclusion from the proceedings and meets the conditions for participation in the procedure. The contractor whose tender will be the highest rated (assessed as the most advantageous), in order to demonstrate the lack of grounds for excluding the contractor from the proceedings referred to in point 2 of Chapter V of the TOR, will be requested to submit the following declarations and-documents (valid as of the date of submitting statements or documents):

a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting offers;

b) statements of the contractor about the lack of a final court decision or final administrative decision on resignation from paymenttaxes, fees or social security or health insurance premiums, or in the event of a verdict or decision confirming payment of such claims together with interest or fines, or a binding agreement on repayment of such claims; - c) the contractor's statement about the lack of a ruling on him as a precautionary measureto apply for public orders.

In the case of joint application for contractors, documents and statements in the scope of sub-point 3), each of the contractors jointly applying for the order is submitted.

4. Documents submitted by a contractor having its registered office or place of residence outside the territory of the Republic of Poland are specified in - 7 and - 8 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the procurement procedure.

5. In order to confirm the lack of grounds for excluding the contractor from the proceedings referred to in art. 24 sec. 1 point 23 of the Public Procurement Law, the contractor submits, in accordance with the content of art. 24 sec. 11 of the Public Procurement Law (within 3 days from the date of placing by the ordering party on the internet siteinformation from the opening of offers, i.e. information referred to in art. 86 par. 5 of the Public Procurement Law), a declaration on belonging to or lack of belonging to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. Along with the submission of the statement, the contractor may provide evidence that links with another contractor do not lead to distortion of competition in the procurement proceduresubmission. In the event of a joint application by contractors, the subject statement shall be submitted by each of the contractors jointly applying for the order.
