Development of design documentation for roof repairs at the eastern stand of the Municipal Stadium in Rzesz-w

Subject procurement is to develop project documentation for the repair of the roof covering and sealing of the drawbarof concrete elements and connections at the eastern stand of the Municipal Stadium in Rzesz-w.

The subject of the order is to prepare project documentation for the repair of the roofing and sealing of expansion joints and connections of concrete elements at the eastern stand of the Municipal Stadium in Rzesz-w.

The task is carried out as part of investment task: "DevelopmentThe area of the roof is a spatial truss system with a cover made of transparent polycarbonate panels. The material scope of the project documentation includes:

1) preparation of the application for the notification of the intention to carry out construction works not requiring a building permit for 2 copies,

2 ) detailed design for the submission of 4 copies robot,

3) itemsar robot 3 copies,

4) investor's cost estimate with a table of integrated elements 2 copies,

5) Technical specification of execution and acceptance of construction works 3 copies,

6) Electronic version on CD in a format read by Acrobat Reader 1 pcs. CD.

The executive design should contain detailed repair solutions. The adopted design solutions will be subject to agreement withEmployer and user of the object. A detailed description of the subject of the order is specified in the graphic attachments and the contract template together with the attachments.

1. These proceedings are conducted pursuant to art. 24aa Pzp.

2. Contractors who are not subject to exclusion under Art. 24 paragraph 1 and paragraph 5. points 2, 4 Public Procurement Law and meet the conditions for participation in the proceedings

3. Dock. and lighting which each Contractor submits with the offer:

3.1. Completed and signed form The offer constituting the attachment to the ToR.

3.2. Powers of attorney if required to represent the Contractor in the event that:

a) The Contractor is represented by a proxy,

b) The bid is submitted by Contractors jointly applying for a public contract with the content required in art. 23 clause 2 Public Procurement Law (also applies tocivil society workers).

3.3. Dock. resulting in the authorization to sign the offer.

3.4. OSW. in the form of a single European order document.

3.5. The Employer will download from the electronic registers of the Central Register of Economic Activity and / or the National Court Register (KRS) doc. registration regarding the Contractor. In the case of Contractors who are not subject to entry to the indicated registers in the offer form should beprovide the address of the website from which the Purchaser will download from publicly available and free databases doc. or attach to the doc. offer indicating the right to sign it. In the case of foreign Contractors, a Polish translation of these documents should be attached to the offer.

3.6. Obligation of another entity to make available the necessary resources for implementation purposesand orders, as long as the Contractors refer to the resources of other entities, in accordance with art. 22a Pzp.

4. Within 3 days of posting on the website by the Employer information referred to in art. 86 paragraph 5 of the Public Procurement Law, Contractors submit, pursuant to the content of art. 24 section 11 Public Procurement Law, on belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 Pzp.

5. The contractor whose tender has been rated the highest is required to submit within the time limit set by the Employer not less than 10 current days as at the date of submitting the statement. or doc. confirming the circumstances referred to in art. 25 paragraph 1 Pzp:

5.1. In order to confirm the lack of grounds for exclusion from participation in the procedure pursuant to art. 24 paragraph 1 Public Procurement Law:

1) information from KRK wthe scope specified in art. 24 paragraph 1 point 13, 14 and 21 Public Procurement Law issued not earlier than 6 months before the deadline for submitting bids;

2) statement Contractor about the lack of issuing a final court judgment against him or a final administrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the case of issuing such a judgment or doc.confirming the payment of these debts together with any interest or fines or concluding a binding agreement on the repayment of these debts;

3) statement Contractors about the lack of a decision against him as a precautionary measure forbidding public procurement.

5.2. In order to confirm compliance with the conditions for participation in the proceedings, the Contractor submits the documents referred toand in points III.1.1 and III.1.3 of the notice.

5.3. The Employer requests from the Contractor, which relies on the abilities or situation of other entities on the principles set out in art. 22a of the PPL, submitting with respect to these entities the documents indicated in items 5.1.

6. Condition regarding competences and authorizations to conduct a specific professional activity, provided that it results from separate provisions: Employer not approxspecifies the conditions in this respect.

We do not specify the minimum level, if any, required standards.

The Purchaser does not specify any conditions in this respect

1. The Awarding Entity does not envisage the use of an electronic auction, a framework agreement or the establishment of a dynamic purchasing system.

2. The Contractor is obliged to lodge a security deposit in the amount of: PLN 1, 500.00 on the terms set out in the ToR.

3. Information forand Contractors having their registered office or place of residence outside the territory of the Republic of Poland:

3.1. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the document referred to in section III.1.1, point 5.1.1, it submits information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative authority of the country, in which Performthe lawyer has his registered office or place of residence, or the place of residence is the person whose information or doc. to the extent specified in art. 24 paragraph 1 points 13, 14 and 21.

3.2. The document referred to in item 3.1 should be issued not earlier than 6 months before the deadline for submitting bids.

3.3. If in the country in which the Contractor has its registered office or place of residence or place of residence, there is aboth whose doc. applies, the documents referred to in item 3.1 do not appear to be replaced by the documents containing, respectively, Contractor that an indication of the person or persons authorized to represent him, or people whose doc. was to relate, submitted to a notary public or before a judicial, administrative or professional or economic self-government body competent for the seator the place of residence of the Contractor or the place of residence of that person. The provision of point 3.2. shall apply accordingly.

3.4. Contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whom the doc. indicated in section III.1.1 point 5.1.1 submits the document referred to in point 3.1 to the extent specified in art. 24 paragraph 1 points 14 and 21. If in the country inwhose place of residence is the person whose doc. was to apply, no such doc appears, it is replaced by containing that person filed before a notary public or before a judicial, administrative or professional or economic self-government body competent for the place of residence of that person. The provision of point 3.2. shall apply accordingly.

4. The Purchaser specifies that termin binding offer is 60 days.
