1. Subject of the orderof this tender procedure is the delivery of cleaners and economic articles for the Central Clinical Hospital of the Medical University of Lodz at ul. Pomorska 251. according to the assortment and quantities specified in the Bidding Form and assortment price attached as Annexes 1 and 2 to SIWZ

Subject of the order niniOur tender procedure is Delivery of cleaners and business items for the Central Clinical Hospital of the Medical University of Lodz at ul. Pomorska 251. in accordance with the assortment and quantities specified in the Bidding Form and assortment price attached as Annexes 1 and 2 to the ToR - Documents confirming that the contractor is insured against liabilitywithin the scope of the activity carried out related to the subject of the order for the guarantee sum specified by the Employer. 1. As regards the confirmation of not being subject to exclusion pursuant to art. 24 sec. 1 point 23 of the Act, the Contractor shall submit: - the Contractor, within 3 days from the date of posting on the website the information referred to in art. 86 par. 5 (ie the amount that the Ordering Party diesfor earmarking for financing the order; companies and addresses of contractors who submitted offers on time; price, date of order's execution, warranty period and payment conditions included in offers), provide the orderer with a declaration of belonging or non-affiliation to the same capital group, referred to in paragraph 1 item 23. Along with submitting the statement, the contractor may beforeprovide evidence that connections with another contractor do not lead to distortion of competition in the contract award procedure (according to Annex 5 to the ToR)

2. The contracting authority provides for the application of the possibility specified in art. 24aa of the Public Procurement Law

In the present proceedings, the Awarding Entity will evaluate offers and then examine whether the contractor whose tender has been rated the highest is not subject to exclusion andz meets the conditions for participation in the proceedings. If the Contractor will evade the conclusion of the contract, the Ordering Party will examine whether the contractor who submitted the highest-rated tender of the remaining offers is not subject to exclusion and meets the conditions for participation in the procedure. The ordering party requires a bid bond of PLN 33, 330.00 (say: thirty one thousand three hundred trPLN 0.20) in accordance with art. 45 of the Public Procurement Law - applies to all packages). The bid bond for individual packages was defined in Section II.2 - Information on parts.

4. The Contractor is bound by the submitted offer for a period of 60 days. The period begins with the expiry of the deadline for submitting bids.

5. Description of the manner of preparing the offer is specified in the ToR.

Proceedings are conducted in the form of an unlimited tender in accordance with art. 39 above Act. based on Article. 6 par. 1 lit. c RODO.

4. The recipients of your personal data will be persons or entities to whom the documentation of proceedings based on art. 8 and art. 96 para. 3 of the Act of 29.1.2004. Public Procurement Law (i.e.. from 2018 item 1986 with later amendments), hereinafter referred to as "Pzp", as well as entities authorized under contracts for entrusting the processing of personal data (in particular to entities supporting the administrator in the organization of public procurement proceedings).
