1. The subject of the order are deliveries of oncological and various drugs specified in parts for the Clinical Hospital of the Voivodship No. 1 for them. Fryderyk Chopin in Rzesz-w.

point II.2.7) regarding the program, indicates: 12 months, the following should be provided:, counting from the date of signing the contract.for lower orders, a change to the contract requires an annex, except that the Employer shall unilaterally notify the Contractor of the proposed extension of the contract before the deadline for which it was concluded.

Due to the program, i.e. the limited number of characters in the notice in section VI.3, Order provides the following information in this section:

Contractors who are not eligible may apply for the contractthey are foreclosure; meet the conditions for participation in the proceedings regarding:

a) the competence or authority to conduct a specific professional activity, provided that it results from separate provisions. The Contractor will meet the condition if he proves that he has documents confirming the competence or authorization to conduct a specific professional activity, i.e. a concession, permit or license for running wholesalersand pharmaceutical in the field of trade in medicinal products or in the case of a contractor running a consignment warehouse - permission to operate a consignment warehouse. Note: Contractors may jointly apply for the contract. Due to the program, the details are described in the ToR. 1. Valid as at the day of submission of tenders, a statement in the form of a single document (EAT), to the extent specified by the Employer.

3. Each of the Contractors who jointly apply for the order submits JEDZ.

The attached document should be completed in accordance with the Instruction for completing the Single European Procurement Document (JEDZ) form available on the Public Procurement Office website at www.uzp.gov.pl/baza-wiedzy/prawo-zamowien-publicznych-regulacje/prawo-krajowe/jednolity-europejski-dokument-zamycji and in accordance with Annex 3 to the ToR. The Single European Order Document (EAT) must be in the form of an electronic document, signed by a qualified electronic signature, prepared and forwarded to the Employer using electronic means of communication inof the Act of 18.7.2002 (Journal of Laws of 2017, item 1219) on the provision of electronic services. Instructions for preparing and submitting the EAT document are in Chapter VIISIWZ.

If the person representing the Contractor in the award procedure. the order is not indicated as authorized to represent it in theproper register or business register, a power of attorney is required. The power of attorney should indicate the right to represent the entity / Contractor in the procedure for granting an order. Public electronic auction is not expected.

We would like to inform you in accordance with art. 24aa of the Public Procurement Law, that will apply the reverse procedure.

The Employer does not set requirements in this respect, whichThe Contractor is obliged to demonstrate compliance with them in a special way.

I. Statements and documents submitted at the request of the Employer:

A. To demonstrate that there are no grounds, exclude. from the procurement procedure:

1. Information from the KRK to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act.

2. Certificate of the competent Head of Tax Office confirming that the Contractor is not in arrears with payment afterdonation or other document.

3. Certificate of the relevant local organizational unit of ZUS or KRUS or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions.

4. Copy from the relevant register or CEIDG.

5. Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding his default with uitaxation of fees or social security or health insurance contributions or in the event of a judgment or documentary decision confirming the payment of such receivables together with any interest or fines, or the conclusion of a binding agreement on the repayment of these receivables.

6. Contractor`s declaration of no decision against him as a precautionary measurewhen applying for an order public.

7. Contractor`s declaration of non-payment of local taxes and fees, referred to in the Act of 12.1.1991 on local taxes and fees.

8. Contractors having their registered office or place of residence outside the territory of the Republic of Poland submit documents in accordance with -7 of Regulation

9. Contractors based in the territory of the Republic of Poland submit documents in accordance with - 8 of Regulation

10. Behindreferring to - 9 para. 3 Regulation does not require Contractors to submit documents regarding the Subcontractor to whom he intends to entrust the performance of part of the contract, and who is not an entity on whose abilities or situation the Contractor relies on the principles set out in Article 22a Pzp

11. The Employer requests from the Contractor, which relies on the abilities or situation of other entities on the principles set out inArt. 22a of the Public Procurement Law, presenting in relation to these entities the documents listed in point VI.A.6.1 6.7 of the Terms of Reference (not applicable in this proceeding).

B. In order to demonstrate compliance with the condition of participation in the proceedings regarding competence or entitlements to conduct specific professional activities, provided that it results from separate provisions, the Employer requires the presentation of a document confirming competenceentities or authorizations to conduct specific professional activities, i.e. concessions, permits or licenses for operating a pharmaceutical warehouse in the field of trade in medicinal products or, in the case of the Contractor operating a consignment warehouse, permission to operate a consignment warehouse.

C. The Employer requires that the items offered be ordered: meet the requirements of the Form (s)offer price / -ch;

2. in parts from No. 1 to No. 16, they were medicinal products authorized in accordance with the Act of 6.9.2001 Pharmaceutical Law, with the exclusion of art. 4 paragraph 1 and 2 of the said Act.

To confirm compliance with the above requirements of the Employer, the Contractor shall, at the request of the Employer, submit a statement according to the model in Annex 5 to the ToR (original).

III. Content of the Information Clause of Art. 13 GDPR used by the Employer for the purpose related to the procedure for awarding a public contract is presented in Annex 8 to the ToR, which is an integral part thereof.
