CPV Description: Supply of equipment for coronary angiography and angioplasty Angioplasty supplies. The subject of this order is the successive delivery of equipment for coronary angiography and coronary angioplasty, specified in detail in the Assortment form - Annex 2 to the ToR within 24 months of the order being awarded.

2. The bid bond can be paid in one or more of the following forms:

- money,

- bank guarantees or social savings fund guaranteescredit guarantee, however, the surety is always a cash guarantee,

- bank guarantees,

- insurance guarantees,

- sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (consolidated text: Journal of Laws of 2018, item 110).

3. Funds must be paid into the following account:

4. Proof of the bid bond must be attached to the offer or give the Employer when submitting the offer.

5 The Purchaser will refund the deposit under the terms of art. 46 Public Procurement Law Acts from 29.1.2004

Specified in the draft contract - Annex No. 7 to the ToR.

1.WyThe Contractor is obliged to send in an electronic form a preliminary declaration in the form of a single document Attachment No. 6 to the ToR, about not being subject to exclusion from the proceedings within the scope referred to in art. 24 sec. 1 item 12-22 and art. 24 sec. 5 points 1 and 8 of the PPL Act, and the one whose offer will be the highest rated, at the request of the Employer also the following documents confirming these circumstances-ie: -1.1 Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting offers.

1.2 Certificate of the competent head of the tax office confirming that the contractor is not in arrears

With tax payment, issued not earlier than 3 months before the deadline for submission offersor applications for admission to participate in the proceedings, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or payment of installments due to law or suspension in full implementation of the decision-authority.

1.3. Certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of offers or applications for admission to participate inthe provision, or other document confirming that the contractor has entered into an agreement with the competent authority regarding the repayment of such receivables with possible interest or fines, in particular it obtained the legally required exemption, deferral or installment payment of overdue payments or suspension in full of the implementation of the decision of the competent authority. 1.4 Contractor's statement about the lack of a decision-to him, as a precautionary measure, the prohibition of applying for a public order.

In order to submit a statement, the form contained in Annex 3 to the SETC can be used.

1.5 Copy from the relevant register or from central records and business information, if separate regulations require an entry in the register or records of art. 24 sec. 5 point 1 of the Public Procurement Law - 1.6. Contractor, in terminot 3 days from the date of posting on the website information referred to in art. 86 par. 5 of this Act, provides the orderer with a declaration of belonging or non-affiliation to the same capital group referred to in art. 24 sec. 1 item 23 of the Public Procurement Law.

With the submission of a statement, the contractor may provide evidence that links with another contractor do not lead toon the competition in the proceedings for the award of the order.

In order to submit a statement, you can use the form included in Annex 5 to the ToR.

2. The Contractor who refers to the resources of other entities in order to demonstrate the lack of grounds for exclusion and fulfillment to them, to the extent in which they refer to their resources, the conditions for participation in the proceedings:

2.1. also submits uniform documents dboring these entities. Declarations of the entities making the potential submitted on the EAT form should be in the form of an electronic document signed with a qualified electronic signature by each of them in the scope in which they confirm the circumstances referred to in art. 22 ust. 1 Public Procurement Law.

3. The Contractor may entrust the execution of a subcontractor's part of the order. Purchaserrequires the Contractor to indicate part of the order the performance of which is going to be entrusted to the subcontractor and the Subcontractor's submission of subcontractors.

3.1. If the change or resignation from the subcontractor concerns the entity on whose resources the Contractor referred to, on the terms specified in art. 22a paragraph 1 of the Public Procurement Law, in order to demonstrate fulfillment of the conditions for participation in the proceedings, the Contractor is obliged todemonstrate to the Employer that the proposed other subcontractor or Contractor individually meets them to the extent not less than the subcontractor, on whose resources the Contractor referred during the proceedings for the order.

4. In the case of joint application for contractors, a single document shall be submitted by each of the contractors jointly applying for the order. Carrier's submissionspersons submitting an offer jointly submitted on the EAT form should be in the form of an electronic document, signed with a qualified electronic signature by each of them in the scope in which they confirm the circumstances referred to in art. 22 ust. 1 of the Public Procurement Law. These documents confirm compliance with the conditions of participation and no grounds for exclusion to the extent that each of the contractorsdemonstrates fulfillment of the conditions for participation in the proceedings and no grounds for exclusion. - Form and manner of submitting a single document (EAT) according to the rules described in Chapter VII, points 1-6 of the ToR.
