The subject of the order is the successive delivery of hemostatic materials for the Hospital Pharmacy of the Provincial Integrated Hospital in Kielce.

A detailed description of the subject of the order and the quantity is set out in Annex 2 to the ToR, assortment price form. Information regarding the principles of delivery, warranty and remuneration is included in Annex 3 to the ToR, specimen contract.

Contractors may apply for the contract for which there are no grounds for exclusion from the procedure pursuant to art. 24 ust. 1 and meet the following conditions, i.e. have the competence or rights to conduct specific professional activities, provided that it results from separate provisions understood as the Contractor has a current permit to conduct business activity running a pharmaceutical warehouse in the field of supplying medicinal entities (managing a consignment warehouse) ) manufacturingadministration or import of medicinal products in accordance with the provisions of the Pharmaceutical Law of 6.9.2001 (Journal of Laws of 2019, item 499) or other authorization, administrative decision issued on the basis of applicable law, on the basis of which the Contractor provides deliveries, services in the scope of the said order, if the turnover of the assortment in a given package requires the abovementionedpermits.

Contractor to confirm compliance with the abovementioned the condition should be accompanied by a current authorization to conduct business activity running a pharmaceutical warehouse in the field of supplying medicinal entities / operating a consignment warehouse / manufacturing or importing medicinal products in accordance with the provisions of the Act of 6.9.2001 Pharmaceutical Law (Journal of Laws of 2019, Pos. 499) or other authorization / administrative decision issued on the basis of applicable law, on the basis of which the Contractor provides deliveries in the scope of the contract in the event that trade in the assortment in a given package requires the abovementioned Contractor's permissions.

Note: In the case of Contractors jointly applying for the award of the contract (in particular, members of the consorcjum, partners of a civil law partnership) a concession may be submitted by 1 Contractor who independently fulfills the conditions for participation in the procedure or all Contractors jointly, who meet the conditions for participation in the procedure together. permits must be accompanied by an appropriate statement indicating the package to which the declaration relates.

Toinitial confirmation of compliance with the conditions The Contractor whose tender has been rated the highest will be required upon request based on Art. 26 section 1 Public Procurement Law to submit a valid authorization to conduct business activity running a pharmaceutical warehouse in the field of supplying medicinal entities / operating a consignment warehouse / manufacturing or importing medicinal productspursuant to the provisions of the Pharmaceutical Law of 6.9.2001 (Journal of Laws of 2019, item 499) or other authorization / administrative decision issued on the basis of applicable law, on the basis of which the Contractor provides deliveries in the scope of the contract in question, if the turnover of the assortment in a given package requires the above-mentioned Contractor's permits (the Contractor shall applyother information in JEDZ / ESPD in Part IV / C item 12).
