Tenders are invited for International Consultant to review Guidelines for the Programmatic Management of Drug-Resistant Tuberculosis for South Sudan.

Background

UNDP partners with the Global Fund to support and strengthen national responses to HIV, Tuberculosis and malaria. The objective is to ensure access to quality health services for all and enable all those living or affected by the diseases to live healthy, productive lives. The partnership leverages UNDP’s mandate to strengthened institutions to deliver universal access to basic services and rebuilding resilient health services in crisis and post-crisis settings.

· UNDP supports the implementation of Global Fund programmes on an interim basis in countries facing significant capacity constraints, complex emergencies, donor sanctions, or other difficult circumstances;

· Develops national capacity andto gradually reduce the need for UNDP implementation support and to transfer the role of Principal Recipient to national entities when circumstances permit;

· And transforms national responses to the 3 diseases by providing policy and technical support in areas of UNDP expertise and mandate, such as human rights and gender equality, sustainable financing and environmental sustainability, and reaching key populations at higher risk

UNDP has supported the National TB Control Program (NTP) in the development of policy documents, guidelines for the management of Tuberculosis, recording & reporting tools in South Sudan, but all these policy documents and guidelines have not been validated for over 2 years and have now been overtaken by the dynamic progress in the TB diagnosis, management and care. UNDP is seeking an International Consultant to review and update with current global trends the for final validation.

Background

As a background, Tuberculosis (TB) is one of the major causes of morbidity and mortality in South Sudan. The WHO estimates that the incidence of the disease is 146/100, 000 population. This translates to about 17, 600 incident cases. In 2017, TB treatment coverage was 56%. Multi Drug Resistance to TB (MDR/RR-TB) has been identified as an emerging challenge. The estimated MDR/RR-TB incident cases among notified pulmonary TB cases were 270 in 2017. In 2018, only a total of 21 (8%) MDR/RR TB cases were detected. The National TB Programme plans to scale up the use of Gene-Xpert testing in the country from current 10 operational sites to about 13 sites. The country has been implementing the ambulatory model since 2017 with the Short-term Treatment

Regimen (STR) and faces challenges of limited funding, human resource (capacity and availability) and limited infrastructure resulting in low cases diagnosed, delayed treatment initiation and adherence to MDR TB care.



Currently, all the DR-TB Management Information System tools are improvised. The available guidelines and SoPs are in draft form and require updating to bring them in line with global practices and be adapted to the challenging operating environment (CoE) of South Sudan. The NSP 2015-2019, implementation is on course especially around diagnostic capacity. The draft PMDT protocol is not yet validated and it requires an update. There is also a need to have SOPs in order to support the implementation of PMDT activities by the national TB programme and all health facilities. The focus of this assignment is to lead the process for review, updating and finalization of the PMDT protocol and recording and reporting tools and development of job aids and SOPs in close collaboration with all stakeholders. This technical assistance and consultancy is in support of the National TB Programme of South Sudan




Duties and Responsibilities

The purpose of the assignment is to review the South Sudan MDR/RR-TB treatment guidelines and tools as well as align the country’s TB program policy documents and recording and reporting tools in line with current WHO recommendations and country context; working in close collaboration with the national TB Programme, WHO, and all other TB interventions implementing partners in the country.

The main objective of this consultancy shall be to review and update existing draft policy documents and guidelines and recording and reporting tools with special focus to the management of MDR/RR-TB. The international consultant will work with the NTP, WHO, and other TB implementing partners to carry out the following specific tasks:

Phase 1: Review the current policy documents and draft tools:

· Conduct desk review of the South Sudan Ministry of Health NTP draft policy documents and tools as well as all TB and MDR/RR-TB guidelines and tools; in line with current global recommendations and adapted to the South Sudan context.

· Carry out field visits to a few selected sites especially high-volume DR TB diagnosis and management for a situation assessment of current case finding and management activities including the National TB Reference Laboratory to review current practices on MDR/RR-TB diagnosis, follow up and sample transportation and/or referral;

· Review and update the Childhood TB Guidelines, MDR/RR TB guidelines (PMDT and treatment guidelines) including the recording and reporting tools; the commodity requisition form

· Develop job aids and algorithms in an “easy-to-read” format to guide utilization of the developed materials (PMDT, recording and reporting tools)

· Share developed materials with the NTP and the UNDP for circulation to stakeholders for input.

· Review, update and finalize the existing draft policy documents in line with WHO recommendations and the End TB Strategy of 90:90:90 and utilizing the findings and input from stakeholders

· Facilitate the validation and dissemination of the finalized documents during a validation meeting with all key stakeholders in South Sudan

Phase 2: Capacity Building:

Develop training and sensitization materials in line with the PMDT guidelines developed

Conduct sensitization of frontline health workers in the 3 high volume health facilities implementing DR TB diagnosis and treatment on utilization of the guidelines and tools.

Deliverables.

Phase 1: Reviewing the guidelines: Day 1 – 7

· Submit an inception report detailing the approach and expected outputs/outcomes

· Submit drafts of reviewed and updated guidelines and recording and reporting tools

· Submit updated Algorithms and SoPs for DR-TB

Phase 2: Capacity Building Training: Day 6-14

· Clinicians from the three DR-TB treatment facilities sensitized/mentored on DR-TB management and recording and reporting

· Submit updated DR TB commodities requisition Form

· Submit a final consultancy report

Required Skills and Experience

Education:

· Medical degree in medicine with post-graduate specialization in Public Health, Internal medicine or a related field.

· Postgraduate training or diploma in DR-TB management, TB or HIV/AIDS management is an added advantage.

Expereience:

· Minimum 5 years of experience in Tuberculosis management of which at least two years in Programmatic Management of Drug Resistant Tuberculosis.

· Familiar with the World Health Organization recommended guidelines for the management of Tuberculosis.

Language:

· Excellent oral and written English.

Other:

· The consultant will be based in Juba on a full-time basis for a period of 14 days (2 weeks)

· The consultant will report to the Global Fund Coordination Advisor, who will review progress and will certify delivery of outputs
