Background

According to the Asia-epidemic modelling data, the level of HIV new infection is decreasing trend in Myanmar from 2000 to 2020. Among risk groups, low risk women contribute to 24% which is the second highest % of new infection in Myanmar in 2015. Reaching HIV services to low risk women can be done by providing PMTCT services.

Mother-to-child transmission (MTCT) refers to a condition where HIV infection is transmitted from an HIV infected mother to her child during pregnancy, labour, delivery or breastfeeding. The prevention of mother-to-child transmission (PMTCT) is a highly effective intervention with four basic prongs: prevention of primary infection among women; prevention of unintended pregnancies among HIV positive women; provision of specific interventions to reduce the risk of mother-to-child transmission; and provision of care, treatment and support to HIV infected women, their infants and families (WHO 2007; WHO 2010a). PMTCT programmes therefore cover a range of interventions, including improved antenatal services, HIV counselling and testing for pregnant women, antiretroviral drug prophylaxis for HIV positive pregnant women and new-borns, referral to support groups, and counselling on options for safer infant feeding practices. Comprehensive PMTCT programs also include continued follow-up and treatment for HIV positive mothers and their children, especially for the first 18 months of the child-s life.

Prevention of Mother to child transmission of HIV programme started only in two pilot townships in Myanmar in 2001. By the end of 2018, due to gradual expansion of PMTCT services in Myanmar, now 321 townships out of 330 townships are implementing PMTCT programme. MOHS would like to scale up PMTCT programme in all townships in coming years. As PMTCT services are being provided accordingly to global guideline in many townships, HIV prevalence among pregnant women has been decreasing and it is at 0.54 % in 2018. The PMTCT programme achievement can help to achieve ending AIDS as a public health threat by 2030.

At the 10th PPTCT task force meeting in Beijing, China on 15-17 September 2015, Myanmar has committed to achieve the elimination of mother to child transmission of HIV by 2025. To achieve this goal, there are many steps to be done before validation of elimination. The first initial step was the discussion with MOHS and the key stakeholders of PMTCT programme to establish the National validation advisory group, committee and working groups. Currently, NAP is conducting the regular PMTCT working group to assist the further step to achieve eMTCT in Myanmar by 2025. National consultation workshops on EMTCT were conducted in 2016 and in March 2019 respectively.

During the PMTCT working group meeting on 17th Aug 2017, MOHS has already endorsed to hire the National Consultant to develop the National eMTCT roadmap. The roadmap can guide advocacy and policy implementation for achieving programme targets of eMTCT by 2025. During the National EMTCT consultation workshop, the EMTCT site selection was endorsed and some detailed plan was also discussed for development of EMTCT roadmap. In 2020, the validation process will start at one State and one Region for achieving eMTCT. Therefore, UNICEF was assigned as a lead agency to support MOHS with the development of the National eMTCT roadmap in close collaboration with WHO and other PMTCT working group members.

The purposes of developing National eMTCT roadmap and guideline is:

To have clear step by step plan for implementation towards eMTCT as well as for validation process
To have the standardized National roadmap and guideline for achieving eMTCT by 2025
To use the global validation tools in the road map
To guide the working group based on the thematic area
Purpose of the Consultancy

This consultancy will facilitate to the development of a National roadmap and guidelines for elimination of mother to child transmission of HIV and syphilis (eMTCT) in English language.

Assignment Tasks and Expected Deliverables

Task 1

Prepare a tentative action plan and proposed timeline for the development of eMTCT roadmap.

The action plan should include a minimum of the following:

Analysing the available data
How to adapt the regional eMTCT framework to Myanmar country context
Expected deliverables and timeline for key activities.
Deliverable 1

Action plan with timeline

Task 2

Develop and submit draft eMTCT roadmap and guideline to UNICEF or its review and then present it to lead PMTCT working group for their inputs and comments if required.

Naypyitaw Travel will be needed for the task 1 and task 2 about 2 times to NPT (with 5 maximum working days). One of the field visit to one PMTCT site might be needed (3 working days).

Draft eMTCT roadmap with meeting minutes of PMTCT working group members

Task 3

Assist and Facilitate the National AIDS Programme, DOPH to conduct National consultation on eMTCT roadmap and eMTCT guideline with all key stakeholder of PMTCT programme. Facilitate the workshop about 4 days in NPT.

Task 4

Finalize the National eMTCT roadmap and guideline after incorporation of suggestion by the Ministry of Health and Sports and stakeholders

Task 6

Submit an assignment report at the end of the assignment. The report should consist of, as a minimum:

Executive summary
Background with a clear purpose
Strategies
Activities
Action plan
Challenges and lessons learned
Outcome of the consultancy
Recommendations for next steps and action plan
Annexes containing all the outputs and other relevant documents for the consultancy
Final report dissemination might be happened in Naypyitaw with all key stakeholders about 3 days.

Assignment report, including annexes (no more than 10 pages)

Qualification and specialized knowledge/experience required for the assignment:

The consultant must have the following qualifications to be eligible to apply for the assignment;

A minimum of advanced university degree in Public Health/ Epidemiology or other relevant social sciences
More than three years of relevant professional work experience in HIV/AIDS field, preferably in PMTCT programming
Experience working with the governments, UN or other international development organizations.
Good knowledge and understanding of health system and context in Myanmar.
Experience in doing data analysis and writing policy advocacy notes.
Experience in facilitating participatory and consensus building workshop at National and State/Regional level.
Written and verbal fluency in Myanmar and English, with good analytical report writing skills evident.
Good understanding of National Policy, HIV situation and programme context in Myanmar.
Ability to work without supervision to meet the TOR
