Invitation to Bid for Booklet Printing



1. The Local Government of Maitum, through the GAD FUND intends to apply the sum of P 68, 400.00 being the Approved Budget for the Contract (ABC) to payments under the contract for Booklet Printing for Support to BNSs, Midwives, Nurses, SB on Health & SK Federation President. Bids received in excess of the ABC shall be automatically rejected at bid opening.

2. Bidding will be conducted through open competitive bidding procedures using non-discretionary pass/fail criterion as specified in the Implementing Rules and Regulations (IRR) of Republic Act 9184 (RA 9184), otherwise known as the -œGovernment Procurement Reform Act-.

4. Interested bidders may obtain further information from The Local Government of Maitum, Municipal Bids and Awards Committee Chairman-s Office, at Office of Municipal Budget Officer, Municipal Hall, Maitum, Sarangani Province and inspect the Bidding Documents at the address given below from 8am to 4pm, Mondays to Fridays.

5. A complete set of Bidding Documents may be purchased by interested Bidders from the address below and upon payment of a nonrefundable fee for the Bidding Documents in the amount of P 500.00

6. Bids must be delivered to the address below on or before February 28, 2020, at 9:00am at Municipal Bids and Awards Committee Chairman-s Office, at Office of Municipal Budget Officer, Municipal Hall, Maitum, Sarangani Province. All bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause Bid Security.

7. Bids will be opened in the presence of the bidders- representatives who choose to attend at Kalahi Cidss Office, Municipal Hall Building, Wing A, Maitum, Sarangani Province. Late bids shall not be accepted.

8. The Local Government of Maitum reserves the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.

(a) Eligibility Documents -

CLASS -œA- Documents:

(i) PhilGEPS Certificate of Registration and Membership in accordance with Section 8.5.2 of the IRR, except for foreign bidders participating in the procurement by a Philippine Foreign Service Office or Post, which shall submit their eligibility documents under Section 23.1 of the IRR, provided, that the winning bidder shall register with the PhilGEPS in accordance with section 37.1.4 of the IRR.



(ii) Statement of all its ongoing government and private contracts, including contracts awarded but not yet started, if any, whether similar or not similar in nature and complexity to the contract to be bid; and

Statement of the Bidder-s SLCC similar to the contract to be bid within the last two (2) years with an amount of at least fifty percent (50%) of the ABC.

The two statements required shall indicate for each contract the following:

(ii.1) name of the contract;

(ii.2) date of the contract;

(ii.3) contract duration;

(ii.4) owner-s name and address;

(ii.5) kinds of Goods;

(ii.6) For Statement of Ongoing Contracts - amount of contract and value of outstanding contracts;

(ii.7) For Statement of SLCC - amount of completed contracts, adjusted by the Bidder to current prices using PSA-s consumer price index, if necessary for the purpose of meeting the SLCC requirement;
