NOTE:

1. Price validity shall be for a period of __120__ calendar days;

2. Bids must be properly arranged, tabbed, sealed and labeled;

3. Bidders are entitled to one (1) bid only, otherwise, all bids made shall be automatically be rejected;

4. Properly filled-up Price Quotation and Technical Specification using the attached form;

5. Current Mayor-s Permit;

6. Current & Valid Certificate of PhilGEPS Registration/PhilGEPS Registration Number;

7. Warranty Security equivalent to at least one percent (1%) retention money or a special bank guarantee equivalent to at least one percent (1%) of the total contract price shall be required upon acceptance of delivery

8. Should your company be declared as the Single / Lowest Calculated Bidder (S/LCB), the following documents shall be submitted within five (5) calendar days upon receipt of Post Qualification Notice:

a) Latest Annual Income Tax Return filed through Electronic Filing and Payment Services (EFPS) and must be duly validated with the tax payments made thereon for the preceding Tax year be it on a calendar or fiscal year income (per revenue regulations 3-2005) OR

b) Latest Business Tax Return filed through EFPS duly validated with the tax payments made thereon also refers to the Value Added Tax (VAT) or Percentage Tax Returns covering the previous six (6) months (per Revenue regulations 3-2005)

c) SEC or DTI registration,

d) BIR Registration

e) Warranty Certificate and Warranty Terms and Conditions if applicable.

f) Certificate of After-Sales on service/parts, if applicable (warranty, calibration, consumables, technician, service center, etc.)

g) Original brochures or downloaded version from the manufacturer-s official website for the item to be bided with the name and address of the manufacturer.

h) Notarized Omnibus Sworn Statement;

i) Product demonstration, if deemed necessary.
