Request for Quotation for Food and Accommodation for Capacity Building Food Terminal Operators on Management and Operation.

1. The Department of Agriculture-Regional Field Office 7 (DA-RFO 7), through the General Appropriation Act for CY 2019 intends to apply the sum of Eighty Six Thousand Two Hundred Fifty Pesos (a‚+/- 86, 250.00) being the Approved Budget for the Contract (ABC) to payments under the contract for Food and Accommodation.

2. The Department of Agriculture-Regional Field Office 7 (DA-RFO 7), now invites bids all PhilGEPS Registered Suppliers/ Bidders for Food and Accommodation with the following details:

Food and Accommodation

1st Day

*35 pax Live-in (3 meals & 2 snacks)---------------------1, 500.00/pax

*5 pax Live out (2 meals, dinner & 2 snacks)--------------950.00/pax

2nd day

*40 pax Live out (2 meals, dinner & 2 snacks)-------------750.00/pax

3. The Bidder must submit Mayor-s Permit, PhilGEPS Certificate and Registration Number, Income/ Business Tax Return and Omnibus Statement among other documents required by the office.

4. Interested bidders may obtain further information from Department of Agriculture-Regional Field Office 7 (DA-RFO 7) and inspect the Canvass Forms and other documents at the address given below from 8:00 a.m.- 5:00 p.m.
